Hi Victoria,
I'm reviewing the NOI submitted for Oak Park Erosion Control. Everything looks good, except that you signed the
certification in Part E and Mr. Sherman signed the NOI Certification Form attached in Part D. The same person who signs
the NOI Cert Form needs to sign in Part E, and vice versa. If you'll fill out and email an NOI Cert Form with your
signature by COB tomorrow, I can approve the NOI. Otherwise, I'll have to reject the submission. Please let me know if
you have any questions.
