On behalf of The Fund for Sandy Point North Carolina, LLC (FSP), Quible & Associates, P.C.
(Quible) hereby requests renewal of the NPDES Permit No. NCS000517.
The Permit was originally issued to FSP for the Sandy Point Development project with an
effective date of June 15, 2006 and an expiration on June 14, 2011. The Permit was renewed
in 2012 with an effective date of December 1, 2012 and has an expiration date of November 30,
2017.
Upland construction has not yet begun for this project and there are no proposed changes or
modifications to the previously approved Permit. FSP intends to move forward with the
approved project when economic conditions improve. To date, FSP has undertaken the
following activities on the project since its original approval:
• Constructed a 1,400 linear feet mitigation breakwater associated with submerged
aquatic vegetation (SAV) mitigation;
• Established a riparian wetlands shoreline buffer;
• Constructed two hydraulic spoil disposal areas;
• Excavated the permitted boat channels 1, 2 and 3;
• Constructed six jetties to protect the aforesaid channels; and
• Installed channel markers.
Quible has also monitored and conducted quarterly baseline sampling in two shallow monitoring
wells and in two locations in the Sound for five consecutive years and finished in 2013. The
Water Quality Monitoring Plan associated with the NPDES Permit specifies sampling in order to
assess the impact of the future development on adjacent waterways. Due to upland
construction not yet beginning, sampling was paused. The purpose of the initial 5 years of

testing was to establish a strong baseline. Of course, once upland construction begins,
sampling will resume.
There is no new impervious cover on the project at this time, and no stormwater management
infrastructure exists. The project scope has not changed from that which was permitted. All
existing conditions and proposed development activities, as presented in the 2005 application
package and set forth in the Permit remain unchanged, provided only that the development
timeline has been pushed forward.
When the time comes for upland construction to begin, the project's erosion control plan, which
incorporates numerous BMP's used to minimize pollutants in stormwater runoff from
construction activities, will be implemented and maintained in accordance with the NC DEQ
erosion control permit until construction is complete. Upon completion of site construction and
occupation, FSP will develop and implement an illicit discharge detection and elimination
program for the separate storm sewer system in accordance with the NPDES permit
requirements.

Annual reports in respect to the Permit have been completed and submitted to NC DEQ in a
digital format since 2008 to present. Copies of the submitted reports are available upon
request.
The following is attached and shall be considered part of this NPDES Permit Renewal
Application package:
• Two (2) copies of the signed Renewal Application Form and initialed Supplemental
Information Form;
• Two (2) copies of a current Site Map; and
• Two (2) copies of the summaries of Analytical Monitoring completed since the previous
permit renewal in 2012.
Please note that many checklist items on the Supplemental Information Form are not applicable
to our project since upland construction has not yet begun. In the future, when construction has
begun or is complete, this renewal application package would be much more elaborate.

SUPPLEMENTAL INFORMATION REQUIRED FOR RENEWAL OF INDIVIDUAL
NPDES STORMWATER PERMIT
Two copies of each of the following shall accompany this submittal in order for the application
to be considered complete:
(Do not submit the site Stormwater Pollution Prevention Plan)
Initials
�Ll 1. A current Site Map from the Stormwater Pollution Prevention Plan. The location of
industrial activities (including storage of materials, disposal areas, process areas and
loading and unloading areas), drainage structures, drainage areas for each outfall,
building locations and impervious surfaces should be clearly noted.
'COLT 2. A summary of Analytical Monitoring results during the term of the existing permit
(if your permit required analytical sampling). Do not submit individual lab reports.
The summary can consist of a table including such items as outfall number,
parameters sampled, lab results, date sampled, and storm event data.
N/A 3. A summary of the Visual Monitoring results. Do not submit individual monitoring
reports. The summary can consist of a table including such items as outfall number,
parameters surveyed, observations, and date monitoring conducted.
N/A 4. A summary of the Best Management Practices utilized at the permitted facility.
Summary should consist of a short narrative description of each BMP's in place at
the facility. If the implementation of any BMP's is planned, please include
information on these BMP's.
N/A 5. A short narrative describing any significant changes in industrial activities at the
permitted facility. Significant changes could include the addition or deletion of
work processes, changes in material handling practices, changes in material storage
practices, and/or changes in the raw materials used by the facility.

On August 7, 2012, 1 conducted the quarterly baseline sampling in our two shallow monitoring
wells and in two locations in the Sound. Sampling parameters include: Dissolved Oxygen,
Salinity, Turbidity, Temperature, Fecal Coliform, Total Phosphorous, Total Kjeldahl Nitrogen,
Total Nitrogen and Potassium. The Water Quality Monitoring Plan associated with the NPDES
Permit specifies sampling of each of these parameters in order to assess the impact of the
future development on adjacent waterways.
The following sheets include a summary of the sampling results from this round of sampling as
well as the prior rounds of sampling. In addition, I have enclosed a copy of the Environmental
Chemists, Inc laboratory results for Fecal Coliform, Total Phosphorous, Total Kjeldahl Nitrogen,
Total Nitrogen and Potassium. The other parameters including Dissolved Oxygen, Salinity,
Turbidity and Temperature were field tested with a laboratory quality monitoring device.
We will continue to send you a copy of all NPDES sampling results and we will keep a copy in
our files. It is required that copies of this information be made available to NCDENR upon
request, but we are not required to submit this information to anyone unless there is a specific

Quarterly Baseline Sampling (February 12, 2013)

On February 12, 2013, 1 conducted the quarterly baseline sampling in our two shallow
monitoring wells and in two locations in the Sound. Sampling parameters include: Dissolved
Oxygen, Salinity, Turbidity, Temperature, Fecal Coliform, Total Phosphorous, Total Kjeldahl
Nitrogen, Total Nitrogen and Potassium. The Water Quality Monitoring Plan associated with the
NPDES Permit specifies sampling of each of these parameters in order to assess the impact of
the future development on adjacent waterways.
The following sheets include a summary of the sampling results from this round of sampling as
well as the prior rounds of sampling. In addition, I have enclosed a copy of the Environmental
Chemists, Inc laboratory results for Fecal Coliform, Total Phosphorous, Total Kjeldahl Nitrogen,
Total Nitrogen and Potassium. The other parameters including Dissolved Oxygen, Salinity,
Turbidity and Temperature were field tested with a laboratory quality monitoring device.
We will continue to send you a copy of all NPDES sampling results and we will keep a copy in
our files. It is required that copies of this information be made available to NCDENR upon
request, but we are not required to submit this information to anyone unless there is a specific
request. Please contact me if you have any questions regarding this matter.

On August 14, 2013, 1 conducted the quarterly baseline sampling at Monitoring Well # 2 (MW-2)
and Surface Water Location #2 (S-2). Monitoring Well #1 has been damaged and was therefore
not sampled on this occasion. Sampling parameters include: Dissolved Oxygen, Salinity,
Turbidity, Temperature, Fecal Coliform, Total Phosphorous, Total Kjeldahl Nitrogen, Total
Nitrogen and Potassium. The Water Quality Monitoring Plan associated with the NPDES Permit
