The failure was related to incomplete documentation- by the primary analyst, on the analytical records. The samples were received by Pace Analytical and prepared for fecal coliform testing on January 9, 2019- In the process of validating unreported data, the review analvst noted the documentation was incomplete and the results could not be validated. All fields on Pace bench sheets for fecal coliform testing must be completed to provide valid results. The fields that were not documented include: eater bath start temperature_ date read out. time read out, all reagent and material tracking ID numbers: These fields are important information to determine the validity of results. For example, we don't know when the fecal trays were actually read out. They could have been read out before the colonies had an opportunity to growv, We also don't know if the water bath was at the correct temperature for the colonies to grow. For these reasons, Pace Analytical does not believe there was enough documentation to validate and report the sample result. As corrective action, we are increasing our direct qualit}- oversight for the Pacc-Raleigh laboratory, to require remote review of records that directly impact the validity of the data. This allows for a more robust peer review and validation of results - We apologize for any inconvenience this has caused vou.
