The attached issuance letter was sent out last year, but no one ever requested a copy of the renewal COC because we
do not have anything in our Laserfiche Repository for this permit. Once the renewal COC is requested, the COC is
emailed to you, and a copy of that request record and Certificate of Coverage goes into this repository in a folder
numbered like the COC. Please be sure to take care of this as soon as possible so that the facility has a record of
coverage.
Renewal applications were not required. COCs are automatically renewed as long as all fees are paid up at the time of
the General Permit renewal. Please be sure to go to our website for that complete NCG070000 Permit package to make
sure you have all up to date materials.

Can you please confirm that our facility is covered under the general permit? Renewal application was sent on
5/18/2018. However, I have not received the certificate of coverage.
