Dear Permittee:

The North Carolina Division of Water Resources conducted an inspection of the Town of Burnsville DCAR 2018
Annual Report on 6/26/2019. This inspection was conducted to verify that the facility is operating in
compliance with the conditions and limitations specified in Non-discharge Permit No. WQ0002834. The 2018
Annual Report submitted appears to be incomplete and contains data from 2017. In order to complete our
review and determine compliance, the following items need to be submitted.

1. All laboratory data for 2018 residuals. This includes all Fecal Coliform, metals, nutrients and any additional
parameters required by the permit for 2018.
2. Resubmit the PVRF and RSSF forms with applicable data.

Please submit all missing data within 10 business days of receipt of this letter (email is acceptable). If you
