Durham County
On May 23, 2019 and May 29, 2019, Ray Milosh from the Raleigh Regional Office visited your
single-family residence (SFR) wastewater treatment system to evaluate compliance with the
above permit to discharge wastewater. The checked boxes below show what conditions were
noted at your facility:
® Other: The system was visited two times but could not be fully inspected because
there was a dog chained up near the system and no one was home. There were no
obvious signs of failure. The pump and alarm for the Advantex pod were in working
condition. The owner did not contact DWR to schedule a time for an inspection.
