REGAN Secretary LINDA CULPEPPER Director Brenda Black — Hight Family Properties 2605 Rolling Pines Ave Durham, NC 27703 Dear Ms. Black: NORTH CAROLINA Environmental Quality June 12, 2019 Subject: Compliance Evaluation Inspection Single Family Wastewater Treatment System Permit No. NCG550902 Durham County On May 23, 2019, Ray Milosh from the Raleigh Regional Office visited your single-family residence (SFR) wastewater treatment system at 921 Jones Circle to evaluate compliance with the above permit to discharge wastewater. The checked boxes below show what conditions were noted at your facility: ® Failure to analyze the effluent: The effluent that is discharged from your system must be analyzed once each year. See Part I (A) of your permit about his requirement. A list of NC certified laboratories that provide this service was left at your residence during the inspection. Make arrangements for sampling to b carried out within the n xt 3 months, n u mit results to this office within 3 weeks after the sampling has-been done. ® Other: The system is buried except for the chlorinator and the outfall. There were no signs of failure on the surface at the septic tank or sand filter. DWR has records showing that the septic tank was pumped in October 2017. There were tablets in the chlorinator. The system was discharging effluent to a roadside ditch in the front yard. The owner did not respond to the paperwork left at the residence requesting she contact DWR to answer questions.
