Sorry about the confusion. There are several separate entities of the Sam's company. SXCW Properties 11, LLC is the
property owner and Sam's Xpress Car Wash is one of the business entities. Sami Nafisi is the manager for the property
owner, and overall owner of all companies. Sami has granted me signing rights as development manager for all
companies when it comes to the NOI /NOT. I am authorized to sign as an authorized agent.
Do I need to change anything on my submittal. Also, I completed one for a project we have in Wilson, NC. I completed it
the same way. I have not sent the certification form in for either as of yet. I can change if needed. This is the first of
these forms we have completed sense the new rule took place.
Please let me know what I need to correct.

The eNOI submitted for Sam's Express Car Wash construction project in Sanford lists the Permittee Organization as
"SXCW Properties 11, LLC," but you are listed as the Development Manager for "Sams Express Car Wash" on the permit
contact tab. Are you also a responsible officer in the SXCW Properties 11, LLC organization? Or are these two separate
entities? I am inquiring because you signed the certification instead of Sami Nafisi, who is listed as the permittee.
Please let me know as soon as possible so I know whether I can approve this application.
