An information package was left at your residence that requested you contact this office. We do not have a record of you calling or emailing us. A permit violation was observed during the inspection, and therefore this report is a Notice of Violation. Upon inspection and file, the following was observeddetermined: 1. The residence is served by a failed wastewater system. 2. A new permit for a discharging sand -filter system was applied for and issued on June 2, 2017. The system was proposed to discharge in the backyard on the west side of the property to an unnamed tributary of Little Lick Creek, 3. Based on the site visit, the system has not been installed, and the residence is still connected to a failed system. Because the approved system has not been installed in almost two years since the permit was issued, you are violating a condition of your permit. The system should be installed immediately or the Certificate of Occu anev for the residence is subject to revocation by the County. If the system is installed, the Engineering Certification that accompanied the issued permit should be submitted after construction. Please let this office know if you either plan to install the system or wish to have the permit rescinded.
