If you are able to upload the email, perhaps you can print a copy of the final application and just sign the hard copy and email it to me and I can forward that to the guy at the State I have been speaking with. Review of the plans for this project have been completed. The design is adequate to meet the ordinance standards and the plans have been approved. If you have not already done so, please provide an original signed and notarized Financial Responsibility/Ownership form. This document is required prior to the issuance of the Grading/Erosion Control Permit. Please obtain coverage under the permit at the following website (https://edoes.deci.ne.2ov/Forms/NCGO1-NOI). Once coverage is obtained please upload a copy to the project documents in the electronic plan review system.
