I. The most recently issued permit (February 18, 2019) states that The City of Nashville operates a
wastewater collection system consisting of approximately 32.7 miles of gravity sewer, approximately
5.1 miles of forcemain, 13 duplex pump stations. The current permit expires on January 31, 2027. A
renewal application should be submitted at least 180 days prior to expiration.
2. The collection system is classified as a CS-2. A review of the Division's database BIMS (Basinwide
Information Management System) revealed that a Primary Operator in Charge (ORC) has not been
identified. Per permit conditions 11. k "The permittee shall provide documentation of Certified
ORC to the Operator Certification Unit of the DVVR no later than April 2, 219."
A Public Education Program for Fats, Oils and Grease (FOG) should be implemented that includes
listributing education material to residential and non-residential users at least twice per year. Per the
*RC a power point presentation is available on the City's website. However, he was unable to locate
it at the time of the inspection. You are reminded that you are to distribute the education to users at
least twice a year. This was also noted in lite past two compliance evaltiation inspections conducted
,�y the Division. Please review Permit Section L4.
4. The Capital Improvement Plan (CIP) was not available for review during the inspection. Please
review Section 1.5 of the permit for the requirements. The CIP was provided by Jamey Baines via
email on May 17, 2019. You are encouraged to review Permit Condition 1.5.

5. The following pump stations were inspected: Cypress Landing Station, Cottonwood Station,
Wi-twood Station.
a. All stations have working visual and audible alarms.
b. The pump stations had the required signage with the required information and all fencing was
maintained.
c. The back-up generators were tested under load. The generator at the Cottonwood station did not
start at the time of the inspection. Mr. Baines contacted staff immediately to start work on the
generator. Please ensure the back-up generators are tested on a regular basis.
d. Contingency plans were available at each of the pump stations.
6. The following high priority lines were inspected: Stoney Creek/DOC Arial at Industrial Park; Stoney
Creek Sewer Crossing at Washington Street; Nash County Stoney Creek Sewer Crossing at
Washington Street; Calvin's Branch Outfall. All were rust free and appeared to be in good condition.
The fight -of -ways are maintained. Some debris was noted against the Nash County Stoney Creek
Sewer line Crossing at Washington street.
7. A sewer line cleaning program is implemented with cleaning logs available for review. In 2018.,
20,602 linear feet of sewer line were cleaned.
S. Adequate records are being maintained regarding customer complaints, pump station inspections,
high priority and easement inspections.
