ST- �:nROLINA Envlronmental Qualiry May 21, 2019 Subject: Compliance Evaluation Inspection Single Family Wastewater Treatment System 121 Pleasant Dr., Durham, NC 27703 Certificate of Coverage (CoC) NCG551684 Durham County On May 8, 2019, Jason Robinson and Erin Deck from the NC Division of Water Resources visited your single-family residence (SFR) to evaluate compliance of the property's discharging wastewater treatment system that is covered under NPDES General Permit NCG550000 and the Certificate of Coverage (CoC) NCG551684. They spoke with Ms. Wilma Lloyd who was present at the time. An information package was left asking the owner to contact this office. We don't have a record of you contacting this office. Upon a file review the following were observed: • This permit was issued in May 2016 after it was determined that the wastewater treatment system was an unpermitted sandfilter system. Before or around that time, a chlorinator, chlorine contact chamber, and dechlorinator was added to the system. • Durham GIS shows you (Elvis LIoyd) as the owner of this property since November 2015. However, the permit is issued to a previous owner. Please see the checked (®) boxes on the following page more details of the findings of this inspection: I. Z Ownership Change Needed: The current permit is issued to a former owner. Therefore, you are currently illegally discharging without a permit. Please complete and submit the attached "Ownership Change Form" to the address on the form. 2. Lai Treatment tablets missing or are wrong hind: You are responsible for always having chlorine tablets and dechlorination tablets in place. They must be the kind for wastewater treatment and not for swimming pools. Raleigh, NC 27699.1628 the system at the time of the inspection Tablets should always be kept in the tubes and may need to be poked occasionally to ensure they are in contact with the wastewater stream. 3. ® Pumping the septic tank: The permit requires the septic tank to be pumped every 3 to 5 years. A pumping company can check the status periodically and determine when pumping is required. Our files show that the septic tank was pump in August 2015. Therefore. the septic tank should be pumped again before August 2020. 4. 5� Failure to analyze the effluent: The permit requires that the effluent that is discharged from your system is analyzed annually. A list of NC certified laboratories that provide this service was provided to you during the inspection. Please have the effluent analyzed and submit a copy of the records to this office. 5. ® Other: At the time of the inspection, there were several people present cutting trees. There were vehicle track -marks associated with the tree -cutting in the vicinity of the septic tanklsandfilter system. Vehicles should not be driven over or near the system. Please respond to this inspection in writing to the address at the bottom of the first page. Your response should address the five checked boxes above. This response should be separate from the 'Ownership Change Form" that should be completed and sent to the address on that form (attached).
