On May 8, 2019, Jason Robinson and Erin Deck from the NC Division of Water Resources visited your
single-family residence (SFR) to evaluate compliance of the property's discharging wastewater
treatment system that is covered under NPDES General Permit NCG550000 and the Certificate of
Coverage (CoC) NCG551684. They spoke with Ms. Wilma Lloyd who was present at the time. An
information package was left asking the owner to contact this office. We don't have a record of you
contacting this office.
Upon a file review the following were observed:
• This permit was issued in May 2016 after it was determined that the wastewater treatment
system was an unpermitted sandfilter system. Before or around that time, a chlorinator,
chlorine contact chamber, and dechlorinator was added to the system.
• Durham GIS shows you (Elvis LIoyd) as the owner of this property since November 2015.
However, the permit is issued to a previous owner.
Please see the checked (®) boxes on the following page more details of the findings of this
inspection:
I. Z Ownership Change Needed: The current permit is issued to a former owner.
Therefore, you are currently illegally discharging without a permit. Please complete and
submit the attached "Ownership Change Form" to the address on the form.
