The checked boxes below show what conditions were noted at your facility: ® NPDES Permit Name/Owner Change Form: Because your treatment system makes an outlet to waters of the state, it is an activity for which the subject permit is required. To comply with North Carolina General Statute § 143-215.I (a), which requires a person to obtain a permit to make an outlet into the waters of the state, you will need to complete and submit the attached NPDES Permit Name/Ownership Change Form to the Division within 45-days receipt of this letter. ® Other: It has been brought to the attention of this office that the onsite system is a traditional septic system and is now currently failing. After a subsequent file review, it was determined that the soils located on property were deemed unsuitable by Durham County for this type of system and a new discharging wastewater system must be installed. Attached to this inspection report, you will find an Authorization To Construct (ATC) Application. Please complete and mail this form to the address located at the end of the document.
