I have briefly looked over the monitoring well application that you submitted, I additional information for a couple of things: -          We will need the original copy with the “wet” signature of the applicant, without the ”wet” signature we cannot issue the permit. -          I need additional information on the proposed well construction, including interval for the gravel pack, bentonite seal and grout. -          Also under the “Additional Information” Section e. of the permit, let me know if any of these sources are present within 500 feet. Please feel free to call me if you have any questions. Thank you, Brion Brion N. Byers, P.G.
