However, additional information is required before the review may be completed. Please address the items on the attached pages no later than the close of business on May 27, 2019. Please be aware that you are responsible for meeting all requirements set forth in North Carolina rules and regulations. Any oversights that occurred in the review of the subject application package are still the Applicant's responsibility. hi addition, any omissions made in responding to the outstanding items, or failure to provide the additional information on or before the above requested date may result in your application being returned as incomplete. Please reference the subject application number when providing the requested information. All revised and/or additional documentation shall be signed, sealed and dated (where needed), with two paper copies and one electronic copy. Wilson April 26, 2019 Page 2 of 2 1. The application shall be signed in accordance with 15A NCAC 02T .0106(b). The application must be signed by an elected official in the highest level of elected office or other authorized employee. An alternate person may be designated as the signing official if a delegation letter is provided from a person who meets the criteria in 15A NCAC 02T .0106(b). 2. Provide the highest and discharge (or connection to another distribution line) elevations in Table III.1. 3. Ensure that if the elevation difference exceeds 10 feet, air release valves are provided as required by 15A NCAC 02T .0305(h)(5). 4. Provide copies of all relevant easements, road encroachments, floodplain compliance authorization, and ACOE permit approval. For easements, any referenced plats or maps must be provided. 5. Provide documentation of the presence or absence of threatened or endangered species at the project site utilizing information provided by the Department's Natural Heritage Program. 6. Per 15A NCAC 02U .0403, the separation requirements have not been met. At the crossings, the piping and integrity testing procedures must meet water main standards in accordance with 15A NCAC 18C. Address. 7. Per 15A NCAC 02U .0201, the specifications must be signed and sealed by a North Carolina Professional Engineer.
