NC0039152 I11 121 19/04/03 117 18 JCJ Section D: Summary of Finding/Comments (Attach additional sheets of narrative and checklists as necessary) The Inspector, Linda Wiggs, met with Mark Swann the ORC, Jarret Lunch, Scott Roman and Nelson Webb with Ohio Motors. The septic tank was pumped 2/26/2018. Given the age of this system it is suggested the septic tank, filter compartment and pump tank be pumped annually. Increased observation of solids/scum buildup level in the tanks and on the filter, can assist with determining annual pumping needs. De -chlorination tubes are in a confined space. Extending the tubes so the tablets can be safely placed in the tubes is recommended. You may also consider removing the sand bags. Tablets are not designed to be submerged in stagnant water. You should not see green residue at the discharge pipe. There were several cleanout/access type pipes along the wastewater/sandfilter distribution lines. It was unclear why they were placed there, but speculated for past camera work. These cleanout/access pipes were not effectively attached to the distributions lines and could allow dirt and non -wastewater water into the wastewater system. It is suggested these pipes be secured to the distribution lines. Sampling for Fecal Coliform in the adjacent tributary (Paint Fork) upstream and downstream of the wastewater system was suggested. Samples were obtained by Pace Analytical on 4/4/2019 the results were: Paint Fork Upstream - 37 CFU/100ml Paint Fork Downstream - 11 CFU/100ml Page# DocuSign Envelope ID: 7A614D10-09B6-4A2C-9FOA-ACF7E6466543 Permit: NCO039152 Owner - Facility: Ohio Electric Motors WWTP Inspection Date: 04/03/2019 Inspection Type: Compliance Evaluation Operations & Maintenance Yes No NA NE Is the plant generally clean with acceptable housekeeping? 0 ❑ ❑ ❑ Does the facility analyze process control parameters, for ex: MLSS, MCRT, Settleable ❑ ❑ ❑ Solids, pH, DO, Sludge Judge, and other that are applicable?
