Dear Ms. Landreth:
On May 1, 2018 Rebecca Chandler of the North Carolina Division of Water Resources (DWR) Winston-
Salem Regional Office (WSRO) performed an announced routine compliance inspection of the subject
facility's wastewater treatment and surface irrigation system. WSRO staff was accompanied onsite by
Mr. Fred Thomen. A separate, in-office review of the self-monitoring report records for the period
December 2016 to February 2018 was also conducted. The inspection found the subject facility to be
non-compliant with Permit No. W00019095.
Accordingly, the following observations and deficiencies were noted which require your immediate
attention and action:
1. Permit Condition 11.1 states that"The surface irrigation facilities shall be effectively maintained
and operated at all times so that there are no discharges to surface waters, nor any
contravention of groundwater or surface water standards." During the inspection, a discharge
to surface waters was detected at the pipe stream crossing for the system.A crack in the piping,
covered by rip-rap material, caused the discharge. It is unknown how much or how long the
discharge occurred.
It is the understanding of this office that the discharge has been abated. WSRO commends
Mr.Thomen's quick response and repair to the system piping and for the follow-up provided
to DWR staff. Please provide a detailed plan for how the facility will prevent similar incidents
from occurring in the future. Additionally, provide this office with a copy of the permitted
facility's Operation and Maintenance plan which at a minimum should include operational
functions,maintenance schedules,safety measures,and a spill response plan. Maintaining an
Operation and Maintenance plan is required by Permit Condition 111.1.

In addition to the above-mentioned deficiency, the following items of concern also require your attention
and action:
1. WSRO understands that Fred Thomen is currently filling the role of ORC for Colfax Furniture's
wastewater irrigation system. An in-office review of the subject permit showed that Mr.
Thomen is not designated as the ORC for Colfax Furniture. An ORC designation form was
provided to Mr.Thomen during the inspection. Please fill out the ORC designation form listing
an ORC and back-up ORC that hold certification for wastewater drip irrigation systems. This
form should be mailed,faxed,or emailed according to the directions provided on the form.
2. DWR staff noted residuals held in the facility's septic tank were high.Consider having the septic
tank pumped/residuals removed soon. Additionally, please provide receipt records to this
office for the last residual removal event for the system. A reminder that records of residual
removal are required to be maintained for a minimum of five years per Permit Condition IV.7.
3. During an in-office review of self-monitoring data, it was noted that reports from January 2017
and January 2018 were not received by DWR Central Office. However, these reports were
provided during the compliance inspection of the facility. In the future, ensure that three
copies all self-monitoring data is sent to DWR's Raleigh Central Office.

Accordingly,you are requested to provide a written response within thirty(30) calendar days following
receipt of the Notice.Your response should be sent to my attention at the letterhead address and must
include an outline detailing specific plans to remedy these deficiencies and any future violations.
Your immediate attention is greatly appreciated.Additional observations and comments may be found in
