and other conditions set forth in Pails I, II, III, IV, V, attd VI of Gettei al Pei tna No.
NCG140000 as attached.
This certificate of coverage shall become effective April 8, 2019.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.
