On March 28, 2019, Erin Deck and Zach Thomas from the Division of Water Resources (DWR) Raleigh Regional
Office visited your single-family residence (SFR) wastewater treatment system to evaluate compliance with the
above permit to discharge wastewater. The checked boxes below show what conditions were noted at your
facility:
® In compliance: You are reminded to regularly maintain the chlorine disinfection and dechlorination
systems, have the effluent sampled once a year, and have the septic tank pumped out every 3 to 5 years.
Your good record of operation and meeting the permit requirements is highly commended.
® Other: Thank you for taking the time to speak with Erin Deck on April 2, 2019. Please complete the
attached Permit Name/Ownership Change form, and submit the most recent effluent sampling results.

Spoke with Ms. Donna Brown on 41212019. She is Ms. Carol Smith's daughter. Per Donna- Carol has
recently been moved to an assisted living facility. Donna has recently moved into the home and in the
past has helped maintain the system. She stated the tank is pumped every few years and sampling is
done every July. She said she will forward 2018's sample results and 2019 when she has it sampled.
