On February 6, 2019 NCDEQ Land Quality staff inspected the subject facility for compliance with the
State Stormwater Post Construction Permit SW3130302. Based on the inspection and lack of maintenance and
possible oil/diesel release/impacts to the BMPs, the site is not in compliance. Please see the attached inspection
report. Also, as part of the response please evaluate operational changes that can be implemented to prevent
future BMP impacts.

Accordingly, you are directed to respond to this letter in writing Nvithin 15 calendar days of receipt of
this Notice. Your response should be sent to this office at the letterhead address. Pursuant to G.S. 143-215.6A,

Comment: It appears there has been an oil/diesel fuel release that has impacted the basins. All cells must be
cleaned out and replaced with appropriate media to restore to as built conditions. A NOV is
recommended based on site conditions and no maintenance.
