Mason,
All of the old bridge structure, cables, debris, and temporary trestle have been removed.
We had the NCODT inspection divers review the area to confirm all items on the boat
side scan had been addressed. All in -water work is complete except for the additional
rip rap to be placed on the island side of the project. Let me know if you need anymore
information.

praposed replacement af the NC � I flISO Bridge acrass the At�antic Intracoastal Waterway
(AICWj, mile 264.7, at Surf City, Pender County, NC. T�e foilowing stipulations shall �e
adhered to:
a. The Contractar shall submit the plans artd sck�edule of aperation far apprar�al at Ieast 45
days prior to commencemez�t vf work in the waterway. �ne copy of sueh infarmation sha11 be
subzx�itte� cvncurrently to your Resident Engineer, the United States Coast Guard Commander
{dph}; Federal Building, 4th Floor, 431 Crawfard Street; Portszz�v�th, VA 2�744-5[1D4, and the
U. S. Coast Guard Sector Nvrth �araiina at 721 Medi�al Center Dri�ve, Wilmington, NC 284U1.
Tk�e infvrmation shall include a sketch af the waterway; the bridge; the locativn af any
restri�tions that will be piaced in the waterway s�ch as barges, an�hvrs, and anchor lines; the
location and height abn�e mean high water and detailed descriptian of any scaffolding, or
netting; detailed descriptian indicat�ng the plac�ment, type and dimension of any �o#'ferdatns if
used. The schedule should aiso in�it�de the hours of operation and whether vr nv# the et�uipment
wili be remo�ed ai night. The �antractvr shall camply with all pro�isions of the NaVigation
Ru1es �ternativz�al - Inlarid, ar�ailable from the Superintendent af Dacuments, U.S. Go�er�zent
Printing affice, Washington, D.C. The Contractar shall submit tv the Resident Engineer a �o�y
af all correspondence with the Cvast Guard. No de�viatian fram the appro�ed plans and schedule
of vperatian may be rnade unless the modification has pre�io�sly been submitted and appro�ed
by the Coast Guard.
b. All bridge closures far the existing-to-be-replaced bridge shall be requested in writin� to
be recei�ed at this office at least 15 days in ad�ance. If any clasures will exceed three
canse�uti�e days, we must be advised ai least 3� days in adVance sv that we may ma�e the
appropriate marine natif cations. Ti�r�ne restri�tions far the na�vigational traffi� shutdQwn shall be
strictly adhered to. Any request far an extension af th� closure dates stated above s�iall be
forwarded at leas# 30 days in ad�ance to the Coast Guard for appro�al.
c. At no time during the wark will the waterway he clased tv navigativn without prior
appro�al fram the Caast Guard. Yvu ar� required tv maintain close and regular �ontact wfth
Coast Guard Sector North Caralina �a keep them informed to a�ti�vities in waterway at (252) 247-
4570.

d. Barges that are used in the waterway during the project mnst be marked. Enclasure (2}
outiines temporary marking and lighting requirements for barges and structures not part of the
bridge that will be used during constructio�a. If bazge vr flaat an�hvr lines are used, they must be
marked by buoys, wtuch should be lighted. If you should ha�e any questions, regarding lights on
the bazges ar work flvats, please contact the Chief, Waterways Management Section, at (757)
398-6230. Floating equipment shall ha�e a radiotelephone capable of operation from its main
control station in a��ordance with Part 26 of Title 33, Code af Federal Regulations and shall be
monitored during all periods the floating equipment is on station.
e. During the progress of work, while the channel is in aperation, should any material,
mactunery ar eqtupment be Iost, dumped, thrown o�erboard, sunk ar misplaced which may be
danger�us ta or obstruct na�igation, immediate notice shall be given to the Caast Guard and the
obj ect remo�ed with the utmost dispatch. Until remo�val can be effected, the obj ects shall be
praperly maxked in order to prote�t na�igation. Notice to the Coast Guard shall gi�e a
description and location of any such abject and the actian taken ar being taken t� prvtect
na�igation.
f. C]pvn c�mpletion of the propvsed project, an inspection of the waterway bottom shall be
performed to insure that all construction waste materials ha�e been completely remo�ed.
Certification will be required in writing �y a licensed engineer ar licensed suiveyor that the
waterways ha�e not been impaixed and all construction related debris has been cleared from it.
The certification shall include the actual method used ta conduct the inspection. The Cantractor
shall remo�e any bridge related debris, resulting from the current ar prior wark or vc�urren�es,
disco�ered during #his sutvey.
g. Upon campletion of the bridgework, a responsible official of the North Carolina
Deparl�nent of Transpartatian shall �erify as-built clearances and a statement attesting to the
cvrrectness of the clearances shall be forwarded to this office for record purpose. In liet� of
�erification by the abo�e listed afficial, certifi�ation by a Iicensed surveyor or registered
prnfessional engit�eer registered in the State of North Caralina will be accepted.
h. Except as shown on the plans, no dredging, excavation, filling, rip-rap, or other work
affecting the bottoms, shall be done in conjunction with this work.
i. If during the periods af constniction, permanent lighting cannot be maintained operahle,
the fenders of each pier shall be marked with a battery or power operated white light of not less
than 50 flashes per minute and �isible for a range af 2,d�0 yards on 40°/a of the nights of the
year. Generally, a lamp of 20-cancile power will meet these requirements. If necessary to obtain
co�erage required, a light or Iights on the upstream and downstream sides shall be installed. The
piers shall be so mazked until the constructifln has been completed and permanent na�igativnal
lighting has been installed and determined to be operating satisfactorily. Written approval from
the Coast Guard of temporary lighting during periods of canstruction is required. N� existing
bridge navigation lights shall be impaired or blocked during darkness or periods af redu�ed
�isibility.

j. Compliance with the requirements stated herein does not relie�ve the cantractar of the
obligation or responsibility for compliance with the pro�isions of any other law or regularion as
may be under the jurisdiction of the State of Narth Carolina, ar any other federal, state or lo�al
authority ha�ing cvgnizance of any aspe�t of the location, construction or maintenance of said
bridge. It is ad�ised that the Coast Guard can levy monetary ci�vil penalties far �iolations of
bridge regulations and statutes.
k. In a��ordance with Title 33 Code ofFederal Regulations Part 118.25 —Applicativn
procedure, appro�al af lights and athex signals required shall be abtained prior to con.struction.
Applicatian shall he by Ietter ac�ampanied by duplicate sets of drawings showing (1 } the plan
and elevativn of the structure shawing lights and sigrials proposed, and (2} a small scale �icinity
chart showing praposed bridge and all other hridges within I,OUO feet abave or helow the
pmposed bridge.
The affice af the Bridge Program Manager, Fifkh Coast Guard District, shall be notified

(a} The following baxges shall display at night and if pra�ticable in periods vf restricted
�isibiiity the lights described in paragraph (b} of this section:
{1 } Every barge projecting into a buoyed or restricted �hannel.
(2) Ev�ry baxge sa mvoxed that it �reduces the a�ai�able navigable w�idth af any channel
to less than 80 meters.
(3) Bazges moored in group's more than twa barges wide or to a maximum width of
��er 25 meters.
(4) E�ery barge not moored pa.rallel to the bank ar do�k.
{b) Barges des�ribed in paragraph (a) of this section shall cany two unobstructed all-round
white lights of an intensity ta be �isible for at leasi 1 nautical mile and meeting the
technica.l requ.irements as presc�ibed in 33 CFR 84.15.
(c} A barge or group vf barges at anchor or made fast to one ar more mooring buvys ar
ather similar device, in lieu of the provisions of Inland Na�igation Rule 30, may carry
unobstructed all-rflund white lights of an intensity to 1oe visible for at least 1 nautical
mile that meet the requirements af 33 CFR 84.15 and shall be arranged as follaws:
{1) Any hazge that prajects from a group formation shall be lighted on its outboard
corners.
�2) Dn a single hazge moored in water where other vessels normally na�vigate on hath
sides of the barge, lights shall be placed to rnark the corner extremities of the barge.
(3) Dn barges moored in group formatian, moared in water where other �essels normally
na�igate on both sides of the group, lights shall be placed to mark the comer extremities
of the group.
(d) The fallowing are exempt from the requirements of this sectian:
(1) A bazge or group of barges moored in a slip or slough used primarily far znooring
purposes.
(2) A barge or graup vf barges moored behind a pier head.
(3) A barge less than 2� meters in length when moored in a special anoharage area
designated in accardance with 33 CFR 1 �9.1 U.

4. Constructian af th� temporary work platfarms in thte Atlantic
lntracoastal Waisrway shall he in accordance with approved pfan sheets 5, 5, 7 and 8

5. A bridge pratectian system shall be installed and maintained in
good condition by anti at the expense of the owner of the bridge. Said instalfation and
maintenance shall be fvr the safety af navigation and he in accordan�e with pfans
submitted to and a�pro�ed by the District Cvmmander priar to its construction.
fi. Clearance gauges shall be installed and maintained in a govd and
legible candition by and at the expense af the �wner of the bridge. The type of gauges
and the focations in which they are ta be ins#alled will he subm�tted to the Distri�t
Cammander for appra�al.
7. At! parts of the existing tQ-he-repiaced NC 5�INC 2� 0 Bridge and
temporary work p{at��rrns across the Atlan#ic lntracaastal Waterway, mile �fia.7, not
utilixed in the new bridge shall be r�moved in their entirety and the waterway cfeared t�
the satisfaction af the District CQmmander. 5uch remo�al and clearance shal! be
�ompfeted when the ❑istrict Cammander determines that the cans#ruction of the new
bridge, miie 26�.8, has reavhed a paint where su�h action shoulcf he taken.
8. Whe� the praposed bridge is no longer used for transparta#ior�
purpases, it shail E�e removed ir� its entirety ar ta an elevation deemed appropriate by
the District Gommancfer and the waterway cleared to the satisfactian of the District
Cammander. Such remaval and clearance shall �e com�leted by and at the expense of
the owner of the bridge upnn due noti�e from t#�e District Commander.
5. The appro�al hereby granted shali cease and be nutl and �aid
unless cvnstruction a� the bridge is commer�ced wi#hin three years and completed within
five years after the date of this permit.
