This inspection was requested by Mr. Justin Gray with the City of High Point Public Services Department due to concern about several spills at the facility over the past few years. The assistance and cooperation of Mr. Anthony Blakely during the inspection were greatly appreciated. An inspection checklist is attached for your records and the inspection findings are summarized below. This facility is located at 1800 Albertson Road in High Point, Guilford County, North Carolina. Stormwater related to industrial activity is discharged from the site to Kennedy Mill Creek, which is currently classified as Class C waters, in the Yadkin -Pee Dee River Basin. Documentation & Monitoring Review The Stormwater Pollution Prevention Plan (SPPP) that was provided on site contains most of the required elements, but is insufficient and not fully implemented. Please see below for specific areas of the SPPP that need attention: 1. The general location map does not include the latitude and longitude of each of the site's Stormwater Discharge Outfalls (SDOs). The site map does not show drainage features, drainage areas, direction of flow, or impervious surfaces as required by the permit. The site map should also be updated to include the SDO added when Cascade Die Casting purchased adjacent property. 3. Although Mr. Blakely has done an excellent job addressing, recording, and resolving spills, the list of significant spills in the SPPP has not been updated since 2010. The permit requires that a list of all significant spills and leaks occurring during the past three years be kept with the SPPP along with any corrective actions taken to mitigate spill impacts. 4. Outfalls have not been evaluated for the presence of non-stormwater discharges since 2009. Re- certification that outfalls have been evaluated for illicit discharge must occur annually, and the certification statement must be signed in accordance with Part III, Standard Conditions, Section B, Paragraph 5 of the permit. 5. The Spill Prevention and Response Plan (SPRP) requires some attention in order to address the specific needs of this site. The employee training and preventative maintenance/good housekeeping programs also need to be updated with more site -specific details, as discussed during the inspection. Mr. Blakely mentioned that it was his goal to focus on those areas in the near future to prevent and protect against future spills. Cascade Die Casting Group may also find that having dedicated environmental health and safety personnel would be very beneficial in preventing and protecting against the types of spills that have occurred at the facility, as well as maintaining compliance with the stormwater permit. 6. The SPPP has not been updated or amended since 2010. The permit requires the SPPP to be reviewed and updated annually. Analytical monitoring has been performed as required, but no records of analytical monitoring between the years of 2010-2012 were available. Additionally, qualitative monitoring has not been documented since 2010. Both analytical and qualitative monitoring are required to be conducted semi-annually and records of monitoring should be kept with the SPPP for five years after the sampling date, as required by the permit. Site Review The facility has two SDOs, both of which were observed during the inspection. No discharge was observed at the time of inspection. Receiving waters appeared clear and free of foam, oily sheen, or other pollution indicators. Outfall 001 may need some control measures to prevent erosion, such as rip - rap and/or vegetation. Mr. Blakely indicated that an engineering firm working with Cascade Die Casting Group may be able to provide assistance in that matter. All secondary containment was implemented as required and no other problems or discrepancies were noted. Ms. Taylor -Smith was pleased to find that Mr. Blakely has already been very proactive with regards to addressing the above issues. All spills were reported to the State, recorded, and addressed satisfactorily. Mr. Blakely indicated that Cascade Die Casting Group has contracted with an engineering firm, S&ME, to address problems with the facility's stormwater program. A representative from S&ME, Mr. Wayne Watterson, PE, was present during the inspection. Mr. Blakely's dedication to bringing Cascade Die Casting Group back into compliance is greatly appreciated. Cascade Die Casting Group Inc. / Atlantic Division 08/0S/2013 Page 3 of 3 The incomplete and inaccurate SPPP, as well as the insufficient qualitative and analytical monitoring records, are considered violations of the NPDES individual stormwater permit. You are reminded that pursuant to NC General Statute 143-215.6A, the Division may assess civil penalties of up to $25,000 per day, per violation of the permit. Please reply in writing to this NOV within 30 days of receiving it. Your reply should detail corrective actions with implementation schedules for each of the violations identified above. High aluminum levels in 2010 were recorded and CDC took appropriate action as required by permit to address exceedances. CDC is working with an engineering firm (S&ME) to consider additional training on sampling methods or cost effectiveness of outsourcing sampling.
