This letter is written in response to the Notice of Violation (NOV).issued by your. office following a
Compliance Evaluation Inspection conducted on July 12, 2010 at the above referenced facility. The
following addresses the items noted by the inspector:
Observation #1 - Stormwater Pollution Prevention Plan (SPPPI — A stormwater pollution prevention
plan has been developed and reflects the current operations at the facility. An annual review of the plan
is required and this needs to be updated.
Response to Observation #1 — The current SPPP has been reviewed. A reminder for annual
review of the plan has been scheduled.
Observation #2 — Qualitative Monitorine — Qualitative monitoring has been recorded and evaluated for
all values as listed on the state forms.
Response to Observation #2 — No response needed.
Observation #3 — Anal tical Monitorin —Analytical monitoring data was available and reviewed during
the inspection. Copper is currently above the permitted values. Included with this form is information
about field certification and pH monitoring.
Response to Observation #3 - UFP is working to develop a systematic and comprehensive
assessment approach to resolve this item. Possible items to investigate%valuate include:
• Conduct a review of the current sampling procedures and sampling locations in order
to identify sampling irregularities.

• Conduct a comprehensive site review in order to:
o Identify existing and potential source areas,
o Identify potential operational modifications to reduce stormwater exposure,
a Inspect the existing stormwater outfolls,
o Confirm the absence of non-stormwater related discharges to the
stormwater outfolls that could impact the stormwater sampling results,
o Identify potential alternative sources of stormwater impacts from copper
(e.g. air related emissions from surrounding properties),
o Identify appropriate 9MPs to address copper and pH concentrations,
o Identify and evaluate passive stormwater treatment alternatives, and
o Identify and evaluate active stormwater treatment alternatives (if
necessary).
• Review and update the facility Stormwater Pollution Prevention Plan (SPPP) as
needed following the site review.
• Conduct training and obtain the necessary equipment to successfully obtain pH
measurements within the 15 minute hold time while also complying with established
NCDENR and EPA field sampling procedures.
UFP is committed to resolving these compliance concerns and to preserving the quality of the
stormwater discharged from the property. We hope the above responses serve to fully address the

Enclosed is a copy of the Compliance Evaluation inspection (CET) report for.the inspection conducted at
Salisbury Plant 214, on July 12, 2010, by Ms. Barbara Sifford of this Office. The site drains to Town Creek, Class C
water located in the Yadkin River Basin. The assistance of Mn Jeff Quante was greatly appreciated during the
inspection. I also appreciate the opportunity to discuss the compliance issues with you by phone while at the facility.

This report is being issued as a Notice of Violation.(NOV) due to the facility's failure to cornply with the
conditions in the Stormwater Permit. and North Carolina General Statute (G.S.) 143-215.1, these conditions are
detailed in the Stormwater Pollution Prevention Plan, Qualitative Monitoring, Analytical Monitoring and
Permit & Outfalls Sections of the attached inspection report. In summary the following observations were noted
during the Division of Water Quality inspection:

Dear Permittee:
Payment of the required annual administering and compliance monitoring fee of $715.00 for this year has not
been received for the subject permit. This fee is required by Title 15 North Carolina Administrative Code 2H.0105, under
the authority of North Carolina General Statutes 143-215.3(a)(1), (I a) and (1b). Because this fee was not fully paid within
30 days after being billed, this letter initiates action to revoke the subject permit, pursuant to 15 ncac 2H.01 D5(b) (2) (k)
(4), and G.S. 143-215.1 (b) (3).
Effective 60 days from receipt of this notice, subject permit is hereby revoked unless the required Annual
Administering and Compliance Monitoring Fee is received within that time. Discharges without a permit are subject to the
enforcement authority of the Division of Water Quality, Your payment should be sent to:
