Request For Additional Information Dated November 30, 2006
Request 1. -Please indicate the location of the protected buffers as overlays on the site
plan. Please be sure to break out the buffers as Zone 1 and Zone 2 clearly on the site plan.
Reply 1. - I have included 5 site plans at 200 scale and 5 site plane at 5 site plans at 100
scale that show the requested Zone 1 and Zone 2 buffers shown throughout the entire site.
I had originally just showed the buffers that were in the are that the permit is being
submitted for.
Request 2. -Please indicate the location of the grassed swale on your plan sheet. Please
provide a worksheet fro the grassed swale and a signed notarized Operations and
Maintenance agreement.
Reply 2. - I have included arrows on the site plan that show the flow direction of the
grassed swales, which is away from the wetlands. On the Typical Street section of the
submitted drawings, the swales can be seen from a side profile. This detail should assist
in understanding the stormwater system and how it will be laid out with regards to the
grassed swales. As shown on the previous drawings submitted, there will be no grassed
swales within the buffer zone. I have included an additional information sheet which I
have previously submitted explaining the stormwater process around the wetland crossing
area. I have also included a signed and notarized OM agreement for the grassed swales
and level spreaders. I realize that the level spreads OM agreement wasn't requested but I
felt it was better to send it then for it to be requested at a later time.

DRAINAGE PLAN
After reviewing the proposed drainage plan for the Bridgewater North project,
Baldwin and Associates has changed the drainage plan for the area adjacent to
the wetlands crossing. We are now proposing that the four swales pitch away
from the wetlands crossing as well as raising the elevation of the road and
making the road pitch away from the wetlands. There will be no swales inside
the 50' buffer. There only will be sheet flow run off with in the 50' buffer. This
solution should neglect the need for any additional information concerning the
grassed swales (Additional Information Request #4). This revised drainage
solution was submitted and has been approved for a Stormwater permit for
Bridgewater North.
GRASSED SWAGES OPERATION AND MAINTENANCE AGREEMENT
The required maintenance activities of the property owner for permeable pavement installations
include the following:
1. Swales should be stabilized within 14 days of swale construction.
2. At least once annually, remove excess sediment, especially from the upstream edge, to
maintain original contours and grading.
3. At least once annually, repair any erosion and re-grade the swale to ensure that the run-
offflows evenly in a thin sheet through the swale.
4. At least once annually, inspect all vegetation and re-vegetate the Swale to maintain a
dense growth of vegetation.
5. Grassed swales should be mowed at least twice annually to a maximum height of six
inches.

prior to any changes to the system or responsible party.

LEVEL SPREADER OPERATION AND MAINTENANCE AGREEMENT
The level spreader is defined as an elongated, level threshold, designed to diffuse stormwater runoff.
Maintenance activities shall be performed as follows:
1. After construction and until vegetation has been established, level spreader(s) shall be inspected after
every rainfall. Thereafter, level spreader(s) shall be inspected at least every month and more
frequently during the fall season and after heavy rainfall events:
a. Accumulated sediment, leaves, and trash shall be removed, and repairs made if required.
b. Inspect level spreader(s) for evidence of scour, undercutting, settlement of the structure, and
concentrated flows downhill from the level spreader(s).
c. A level elevation shall be maintained across the entire flow-spreading structure at all times.
Repair or replace the level spreader if it is damaged.
d. Mow vegetative cover to a height of six (6) inches and prune plants if they cover over half of the
level spreader surface.
e. Repair eroded areas and replacelreplant dead or damaged vegetation.
2. The contractor should avoid the placement of any material on and prevent construction traffic across
the structure. If the measure is damaged by construction traffic, it shall be repaired immediately.
3. Inspect and repair the collection system (e.g., catch basins, pipes, swales, riprap) four (4) times a year
to maintain proper functioning.
