Hat77s, III, P.E,, of the North Carolina Department of Transportation (NCDOT} for izxapacts assoczated with the new location project identified as U-3615. The 6.3 mile project widens and improves Skeet Ciub Road to a multi-lane facility from SR 1003 {North Main Street) and SR 1820 (Skeet Ciub Road} between US 311 to NC 68 and reconfigures the intersection at North Main Street and Skeet Club Road in High Point, Guiiford County, North Carolina. For canstruction purposes this project has been divided into two sections: U�3615A includes SR 1003 (North Main St�eet) and SR 1820 {Skeet Club Road} between US 311 and east of SR 1818 {Johnston Street). U-3615B includes SR � 820 {Skeet Club Road} from west of SR 181 S(Johnson Street) to NC b8 (East Chester Di7ve). The project area contains tributaries to West Fork Deep River as well as adjacent wetland areas and impoundments in the Cape Fear River Basin {8-Digit CataIoging Unit 03030003}, Total impacts authorized by the permit ir�clude: 1} the permanent placement of fili material into 2,538 linear feet of jurisdictional stream channel, 0.04 acre of open waters, and 0.82 acre of adjacent iiparian wetlands and 2) the temporary placement of fill rnaterial into 1501inear feet of jurisdictional st�eam channel. The above impacts were autnorized based on finai design for U- 3615B and preliminary design for U-3b15A; construction is not authorized on U-3b15A until pei7nit modifications are submitted and approved based on �inal design for ihat section. Compensatory znitigatzon was implemented for the unavoidable impacts for U-3615B by a combination ofpayznent into the North Carolina Ecosystem Enha�cement Program, now known as ihe North Caz•olina Division ofMitigation Services, as well as on-szte pei7nittee responsible mitigation. NCDOT will provide a compensatory rnitigation plan for U-3615A when permit modifications based on final design are requested for this section. _2_ Also reference your peimit modification request letter dated January 17, 2d19, proposing the following; 1) Extending cnlvert pipes into an additional 1 S linear feet of stream channel, and adding 201inear feet of additional bank stabilization at U-3615B Permit Site 1. This worlc would also require an additional 14 linear feet of temporary si�eam impacts. The purpose of the additianal stream impacts wauld be for NCD�T to improve a tai� Iane and lengthen the two previo�osly-authorized 6b-foot reinforced concrete pipes at the Site. This existing turn lane was not included in the original sur�ey, and as such was not accounted for during the original project design. The existing ttai� lane would be updated to meet NCDOT roadway criteria and the previously authorized reinforced concrete pipes would be leng-�hened. This is a design revision, so the two originally permitted 6b-foot pipes have yet to be installed. Guardrail would also be installed due to an approximately 7 foot vertical drop at the end of the pipes. The Coi�s has completed the evaluation of your request and determined that it is appropriate and reasonable, and no pnblic notice is required for this modifcation. Therefore, the permit is modi�ied as listed and shown in the modified drawings in the January I7, 2019 madification request, for a net increase of permanent impacts to streams of 3 8 linear feet (pernnanent loss of 1& linear feet) and a net increase of temporary impacts to streams of 141inear feet. Project U- 3b15 now totals 1) permanent placement of fill material into 2,57b linear feet of jnrisdictional s�ream channel, 0.04 acre of open waters, and 0.82 acre of adjacent riparian wetlands and 2) the temporaay placement of fill material into 164 linear feet of jurisdictional st�•eam channel. In addition, the following special conditions regarding compensatory mitigation has been incoiporated: x) In order to compensate for impacts associated with this permit, mitigatian shall be provided in accordance with the provisions outlined an the most recent versian af the attached Compensatory Mitigation Respansibility Transfer Form.
