Comment: Final 401 closeout inspection. Two minor areas need attention. At the temoorary stream impact adiacent
to the turn around prior to the intake building the hardware cloth at the end of the ditchline needs to be
removed and the bank must downstream needs to be reshaped, plastic removed, and matted. Silt fence in
this area should come out and area should be smoothed and seeded. At ehe culvert outlet at Sta 23+00
the check dam should be removed and the riprap apron is an approved permanent impact. Site has not
other issues other than maintaining and inspecting S&EC measures and ensuring that ditchlines are
vegetated, and properly functioning, in accordance with Stormwater Permit.
