GENERAL PROGRAM INFORMATION
The City of High Point's Pretreatment Program began 2018 monitoring twenty-one significant
industrial users. During 2018 the pretreatment program renewed two SIU discharge permits,
modified two SIU permits and dropped two SIUs. All significant industrial users had at least one
significant industrial user facility inspection during 2018 and the pretreatment program collected
compliance samples in both the spring and fall. There were no new SIUs added to the program,
but Fairmont Metal Finishing and MB Plating were dropped from the Pretreatment Program in
January.
Updated long-term monitoring plans were submitted for both Eastside and Westside in May
2018. A short-term monitoring plan (STMP) was included as part of Westside's long-term
monitoring plan to calculate a new Headworks Analysis due in late 2019 for the upgraded flow
design. The LTMP for Eastside was later modified and submitted via electronic mail on
December 12, 2018 at the request of the State, to add additional pollutants of concern (Cobalt
and Tin) that were not included in the initial LTMP. Further revisions are expected once the
City's laboratory has their ICP-MS in service.
The revised local limits and subsequent Sewer Use Ordinance updates were submitted to
City Hall but were not sent to the City Council in 2018 as expected. City Council is expected to
approve the changes in 2019. Once approved, the Sewer Ordinance will be submitted to NC
DEQ PERCS.
Eastside Wastewater Treatment Plant (WWTP)
The existing Eastside NPDES permit expired on December 31, 2018 and the incinerator
went back in service in November. There were no upsets, interferences, or pass through
incidents confirmed to have been attributed to currently permitted industrial users at the
Eastside WWTP.

Westside WWTP
There were no industrial user caused upsets, interferences, or pass through incidents at the
Westside WWTP. The upgrade project was completed in Spring, 2018. They continue to
operate with an expired NPDES permit. The previous permit expired on April 30, 2014. The
dates listed for the Headworks Analysis (HWA), Long Term Monitoring Plan (LTMP), Industrial
Waste Survey (IWS), Sewer Use Ordinance (SUO), and Enforcement Response Plan (ERP) are
all correct.
Corrections to Information Sheet
The Eastside SIU permitted flow and percentage needs to be corrected. Currently the
information sheet indicates the Eastside permitted SIU flow is 1.14 MGD which computes to
4.38%; however, according to the allocation table, permitted significant industrial users account
for a total flow of 1.210 MGD which equals 4.97% of Eastside's design flow. The NPDES
effective date for Westside is incorrect. The permit the Westside plant is operating under was
effective on August 1, 2013 and was effective until April 2014. The changes are highlighted and
noted in red text on the program information sheets which are included on the following pages.

SIU INFORMATION AND INSPECTION SUMMARY
All significant users had at least one significant industrial user facility inspection during
2018. A summary of their information and inspections are listed below:
WESTSIDE WWTP, NPDES Permit Number NCO024228
There were four (4) industries recognized as significant contributors during 2018 who meet one
or more of the following criteria:
I. Average Daily Flow 25,000 gallons or more.
II. Capability to significantly impact the POTW.
III. Subject to National Pretreatment Standards.
IV. Subject to Local Pretreatment Limits.
SIU SUMMARY
Cascade Die Casting Producer of custom die cast and machined
1800 Albertson Rd aluminum castings; they are subject to 40 CFR
High Point, NC 27261 464.16. They report that their business production
is down compared to 2017. Construction was
completed on the new tool building. Chemical
storage has been moved to building that does not
have drains. There were no violations noted during
the inspection.
2. Dairy Fresh, Inc. This facility is a dairy processor. They are not
1350 W Fairfield Rd subject to categorical regulations but are classified
High Point, NC 27263 as a significant user due to flow greater than
25,000 gallons per day. Their production is down
compared to 2017; however, added new case
washer so number of washes has increased. There
were no violations noted during the inspection, but
production manager was aware pretreatment may
be required if expansion is indicated.

Specialty chemical manufacturer. They are subject
to 40 CFR 414 D (OCPSF). Their production is
unchanged compared to 2017. BOD and TSS
issues were resolved by switching permit to mass -
based limits. There were no violations noted during
the inspection.
Specialty chemical manufacturer. They are
regulated under 40 CFR 414 H (OCPSF). They
report their production volume has increased 15%
compared to 2017. There were no deficiencies
noted or recommendations made during the
inspection.

There were no industries added or dropped from the Pretreatment Program that discharge to
the Westside WWTP.

Although listed below, the City of High Point dropped two industries in 2018 and recognized
seventeen (17) industries as significant contributors who meet one or more of the following
criteria:
I. Average Daily Flow 25,000 gallons or more.
II. Capability to significantly impact the POTW.
III. Subject to National Pretreatment Standards.
IV. Subject to Local Pretreatment Limits.

Producer of polyurethanes and polyacrylates.

noted during the inspection.
2. Cintas Corporation

SUMMARY
This industry was dropped from the program
on January 29, 2018. The facility closed
abruptly in December 2018. City staff were
unable to perform a closing inspection. As a
result, with assistance from the City of
Archdale, the discharge line from the facility
was severed and capped to prevent
unauthorized discharges from the facility.
Currently, there are plans for NCDEQ Solid
Waste and EPA to remediate the property.
This textile facility is not subject to categorical
regulations and is considered Significant
because flow exceeds 25,000 gallons per day.
They reported their production was down
compared to 2017. Sock printing, which does
not generate wastewater, has increased. There
were no violations noted during inspection.
This facility is a dairy processor. They are not
subject to categorical standards but considered
Significant because their discharge flow
exceeds 25,000 gallons per day. They report
their production is up 30% compared to 2017.
They added a second discharge line to
accommodate increased permitted flow
volume, which is split between the two
discharge pipes. No deficiencies noted during
inspection.
This CIU is an organic chemical manufacturer.
They are regulated under 40 CFR 414 G & H.
Production has increased 20% compared to
2017. There were no violations noted during
the inspection.
9. Kersey Valley Landfill This facility is a landfill and not subject to
3940 E Kivett Dr. categorical regulations and considered to be a
High Point, NC 27260 significant industry due to the potential for
causing adverse effects to the POTW. Tonnage
is up compared to 2017. NCDEQ recently
added 1,4-Dioxane to groundwater monitoring.
Expected to close Kersey Valley Road in 2019
to add additional capacity to sustain landfill
approximately 18 additional years. No issues
noted during the inspection.

SUMMARY
This industry was dropped from the program on

(aka Over -the -Counter Medications or OTCs)
and subject to regulation under 40 CFR 439
Subpart D. They reported their overall
production is comparable to 2017. The plans to
replace the pretreatment building continue to
be delayed. There were no deficiencies noted
during the inspection.
This textile mill is not subject to categorical
regulations and considered significant due to
discharge flow exceeds 25,000 gallons per day.
This location is an overflow of production that
occurs at their Centennial Street location,
expanded dry operations. There were no
violations noted during the inspection.
This textile mill is not subject to categorical

This CIU is subject to categorical regulation

This facility fabricates seats for the
transportation industry with Thomas Built
Buses being their primary customer. SYNTEC
shapes, welds, phosphate -coats and powder
coats metal parts. They report no changes in
production compared to 2017. No violations
were noted during their inspection.
16. Teknor Apex — North Carolina
This facility is regulated under Part 414,
3518 Dillon Rd.
Subpart D. They manufacture thermoplastic

polymer pellets. They indicated a slight
increase in production rates compared to 2017.
All vacuum lines are waterless. There were no
violations noted during the inspection.
17. Thomas Built Buses
This CIU is subject to regulation under 40 CFR

18. Ultra Coatings
This CIU is subject to regulation under 40 CFR

cleaning, phosphatizing and powder coating
metal objects. They are a job shop for
companies such as Honda and John Deere
and report no changes in their production rate.
May add additional customer in 2019. There
were no violations noted during the inspection.
19. Zebra Environmental Services
This facility is categorically regulated under 40
901 E Springfield
CFR 437 B, Central Waste Treatment for oily

wastes. They dispose recovered groundwater
contaminated with gasoline or diesel fuel after
phase separation. They report business has
increased compared to 2017. There were no
violations observed during the inspection.

There were no new significant industrial users added to the pretreatment program in 2018
that discharge to the Eastside WWTP; however, as noted above, both Fairmont Metal Finishing
and MB Plating were dropped in early 2018. The City of High Point was unable to perform a
closing inspection on either business. Since both businesses were in the same building and
due to the known chemicals used, with assistance from the City of Archdale, both discharge
lines were capped or plugged to prevent discharge to sewer and incoming water was shut off.
NCDEQ and EPA are working on plans to remediate the property.

WESTSIDE WWTP, NPDES Permit Number NCO024228
INDUSTRY: Cascade Die Casting
NATURE OF VIOLATION: Copper
SUMMARY: Cascade Die Casting was issued a Notice of Violation in September for exceeding
their copper TRC limit and fined $250.00. They submitted a compliance response and found no
process changes, materials or production that would explain the violation and asked for
assistance. They were advised to troubleshoot their treatment system, specifically the pH, due
to elevated levels of other metals analyzed from the same sample. They discovered the in -line
pH probe and final pH meter were malfunctioning. Both were replaced, and they have
implemented procedures to calibrate more than the required once per year and perform more
frequent pH checks.

Dairy Fresh was determined to be in Significant Non -Compliance for the first and second 6-
month periods of 2018. The City of High Point is in the process of escalating enforcement
which is discussed further in Part 4.
INDUSTRY: KAO Specialties Americas, LLC
NATURE OF VIOLATION: BOD
SUMMARY: KAO Specialties Americas, LLC (KSA) was issued a Notice of Non -Compliance in
January and February for daily BOD limit violations. Their permit limits were switched from
concentration -based limits to mass -based limits in March which resolved BOD limit violations.

SIGNIFICANT NON-COMPLIANCE SUMMARY
The City of High Point had one industry in Significant Non -Compliance (SNC) during 2018:
Dairy Fresh
Dairy Fresh was in Significant Non -Compliance for both the first half of 2018 for BOD and
second half of 2018 for BOD and TSS as defined in 15 NCAC 02H .0903(34)(A) and (B) and as
adopted in the City of High Point Sewer Use Ordinance. To resolve the SNC, the City of High
Point is modifying their discharge permit from concentration -based limits to mass -based limits.
Additionally, the city is in the process of drafting a Notice of Escalating Enforcement which
summarizes the BOD and TSS violations but specifically lists on two occasions, the BOD and/or
TSS pollutant loading exceeded the Maximum Allowable Industrial Loading (MAIL) and
consumed the majority (84% BOD, 74% TSS) of the Maximum Allowable Headworks Loading
(MAHL).
Despite the recent upgrade, the current headworks is based on the 6.2 MGD design. Although
there is allocation available, it is less than the amount needed to allocate to the industry to avoid
further violations, based on average loading for both BOD and TSS and still have a safety
factor. Raising their limits would not resolve their problem. Modifying their permit does not
increase their limits but does provide relief while they evaluate options. While violations still
occur, after evaluating the 2018 data as Ibs/day results, it was indicated SNC designation would
be averted.
Specifically, the city will require that they evaluate "Clean in Place" (CIP) cycles and the
possibility of staggering cycles to create a more uniform wastewater stream, evaluate possible
waste reduction practices especially where there is more possibility of raw product to be spilled
or discharged and guarantee the sampler is maintained and the refrigeration is working.

The letter also stipulates fine amounts for violations that occur before December 31, 2019 and
emphasizes that if violations continue, in addition to higher fines the City does have the option
to require the industry to install pretreatment as stipulated in the Sewer Use Ordinance Section
8-2-54.9(1).
The City of High Point is required to make public notification of industrial waste contributors that
have been in Significant Non -Compliance with Categorical Pretreatment Standards. That
obligation was fulfilled on January 26, 2019 in the High Point Enterprise. A copy of the affidavit
of publication is included bn the following page.

The. City of High Point Industrial Pretreatment has received and reviewed the revised plans for
constructing an additional discharge pipe for your facility. Due to a projected 40% increase in
business, this addition is necessary to accommodate expected increases in discharge flow that
exceed the current discharge capabilities. Authorization to Construct is hereby granted in
accordance with Part 1, Item(C)(3) and Part II, Item #26 of Industrial User Permit # 0107. This
Authorization to Construct is independent of the required engineering permits as required by
City of High Point's Planning, Development and Inspection Services Department. For more

As outlined in the plans, the addition includes adding 236 L.F of 6" force main that connects
from the existing wet well to a new manhole to be constructed and tied into the 8" sanitary
sewer main on Irbywood Ave. Additionally, the replacement project will include the addition
two lift shredder pumps inside the wet well. One pump will serve as the primary and the other
as a backup pump. The discharge meter in the flume will be used to limit the gravity flow
discharged through Pipe 001 to 105 gpm for a maximum discharge of 150,000 gallons per day
to the sanitary sewer main on Montgomery Street. The new 6" force main flow, which will
become Pipe 002, will be measured by a separate meter.
Disruption to wastewater collection and treatment will be minimal during this process.
The issuance of this Authorization to Construct does not preclude the permittee from
complying with all statutes, rules, regulations, or ordinances of the City, the State, or the
Federal Government.

If the modifications fail to perform satisfactorily, including the creation of nuisance conditions,
the permittee shall take immediate corrective action and notify the City of High Point.
Construction of additional or replacement pretreatment facilities will require another
Authorization to Construct.
Upon completion, please notify our office that installation is complete. We will coordinate a
time to inspect the system modifications and more importantly, issue a modified permit with

Our office was contacted regarding the non-compliance notice and advised there were errors on the
log. The first was a typo for the data entry for 08:30 am on 5/23/18. The volume should have been
6600 instead of 8800. As explained, we will need verification of the volume before the notice is
rescinded for May 23`d
We have determined the flow volume was miscalculated for June 111h. The calculation included an
entry from June 91h and the latter two entries on June 11th. The calculated flow volume for June 111h
is 12,180 gallons. Therefore, we are rescinding the Notice of Non -Compliance for June 11, 2018. If

inspections. Please provide PERCS copy of follow-up/verifications to ensure that all leftover process

effectively control and document the discharge of wastewater from Significant/Categorical Industrial

PERCS Unit of the Division of Water Resources has reviewed the updated Long Term Monitoring Plans
(LTMPs) for both Eastside and Westside wastewater treatment plants submitted by the City of High Point.
The LTMPs were received by the Division on May 14, 2018.
The review indicates that the updated LTMPs are adequate and meets the requirements needed to
establish a site -specific Headworks Analysis (HWA). Proper implementation of an LTMP is also required
by Part IV, Section D, of your NPDES permit.
1. LTMP Data Summary: Comprehensive Guide, Appendix 4-8 states that sample collection and
laboratory analysis must be done as outlined in LTMP. It is also very important to organize and
summarize your data.
2. Division Review of Data: As your LTMP data is accumulated, organized, and analyzed, the Division
may request to review tabulated data and to see chain of custody forms and laboratory data sheets.
3. LTMP Modifications: Interpretation of monitoring data or changes in operation at your POTW may
indicate that the LTMP needs to be modified; however, please be aware that modifications to your
LTMP must be approved by the Division.

On August 7, 2018, Paul DiMatteo of the Division of Water Resources Winston-Salem Regional Office
conducted a Pretreatment Compliance Inspection at the above referenced wastewater treatment system. The
purpose of this inspection was to determine the effectiveness of the Town's pretreatment program, which
included: reviewing the files, POTW plant performance, sampling and compliance judgement program, and
adherence to the enforcement response plan (ERP)
Background
The City operates one 6.2 MGD facility and one 26 MGD facility, and has 21 significant industrial users
(SIUs), 13 of which are categorical.
POTW Interview
During the most recent semi-annual period (January through June, 2018), the City determined that one
industry, Dairy Fresh (#151), was in significant noncompliance for limits violations. Ms. Hyler said that public
notice would be published in 2019 before the Pretreatment Annual Report (PAR) is submitted. Additionally,
bypass issues with Hunter Farms described in the previous PAR were resolved through an agreement by their
and the town's lawyers earlier this year. All other violations are identified and handled according to the
Enforcement Response Plan.

Pretreatment Program Elements Review
The Headworks Analysis (HWA) renewal was submitted for the Westside plant on April 27, 2017 and
approved on July 26, 2017, and for the Eastside plant on June 5, 2017 and approved on August 10, 2017. The
next HWA will be due for Westside and Eastside on December 1, 2019 and May 31, 2022 respectively.
The City's last Industrial Waste Survey (IWS) was submitted on January 21, 2015 and was approved on
February 5, 2015. The next IWS is due January 31, 2020.
The last Long Term Monitoring Plan (LTMP) for both plants was submitted on May 14, 2018 and
approved on August 8, 2018.
The Sewer Use Ordinance (SUO) was approved on January 10, 2013
The ERP was approved on October 9, 2008.
LTMP Review
The LTMP is being conducted at the proper locations and frequencies.
Industrial User Permit (IUP) File Review
A review of the files for Diary Fresh (#151), Zebra Environmental (#153) and Innospec Performance
Chemicals (#120) revealed that monitoring data was well organized and violations were addressed in a timely
manner consistent with the ERP. The slug control plans were in good order.
The pretreatment compliance inspection was satisfactory. The program is being implemented in an

EPA defines pollutants of concern (POCs) as any pollutant which might reasonably be expected
to be discharged to the POTW in quantities which could pass through or interfere with the POTW,
contaminate the sludge, or jeopardize POTW worker safety and health. One of the criteria to
determine pollutants be included in the LTMP POCs list is if it is limited in significant industrial
user (SIU) permits.
In recent industrial user permit (IUP) reviews, it came to the PERCS Unit's attention that certain
IUP-limited pollutants are not always included in the LTMP as they are not conventional
pollutants that are typically evaluated in the Headworks Analysis. For example, antimony, cobalt,
tin, titanium and vanadium are required to have limits in the Centralized Waste Treatment (40
CFR 437) categorical standard. Not all of them have water quality standards or are on the 126
priority pollutants scan list but they might have adverse impacts on the aquatic life or human
health like other emerging contaminants. After a discussion with the NPDES Complex Permitting
Unit, the decision has been made that these pollutants are required to be added to your LTMP
and monitored at least 3 times every 5 years along with the 126 priority pollutants scan only at
the effluent. If any pollutant is detected at the effluent using the following detection levels (DLs),
the influent should also be monitored.

Subject: End of 2018 Pretreatment Mailing
Dear Pretreatment Professionals:
As we draw to the close of another great pretreatment year, I wanted to provide you an update on some issues,
inform you on this year's upcoming workshops and distribute the Pretreatment Program Info Database sheets.
1) Annual Pretreatment Conference: Due first to Hurricane Florence and then an early winter snow storm, the
annual conference scheduled for Wrightsville Beach was canceled. PT 101 has been rescheduled for January
17, 2019 at the NC -PC Industry Day in Greensboro. Please see the NC -PC web site for further information.
2) Dental Amalgam Rule: A Draft enforcement policy was sent to the NC -PC Mercury Workgroup on November
30. PERCS would like to have the initial comments by January 31, 2019.
With the promulgation of the dental amalgam rules there have been several inquiries regarding removing the
Mercury Minimization Plan (MMP) requirement from NPDES permits. Currently requests to remove the MMP
can be made if a POTW can demonstrate effluent mercury concentrations consistently below 1 ng/I. Future
request could be considered, even if the effluent concentrations are not consistently below 1 ng/I, if a reduction
in influent mercury concentrations can be shown and the POTW can verify that most (>90%) dental users
within their jurisdiction have installed amalgam separators.
3) Rules Review: The EMC approved the rules at the March 8, 2018 meeting. Two public hearings were held in
May. No comments regarding the pretreatment rules were received. The Hearing Officer's report is currently
in draft.
4) NPDES Pollutant Scans: It has been noted that detection levels used for the effluent pollutant scans are
sometime different than those used in the pretreatment long/short term monitoring plans. All of this data is used
by the NPDES permit writer when performing the reasonable potential analysis. Having differing detection
levels may cause the data to show reasonable potential to violate the standard and result in a limit in your
NPDES permit. Please contact your contract lab and make sure the same sufficiently sensitive detection levels
are used for all effluent analysis. The current required detection levels for silver = 1 ug/l; cadmium = 0.5 ug/1
and; selenium = 1 ug/I.
5) Division Review of IUPs: The Division continues implementation of the streamlined permit review process.
After attending the IUP workshop and a thorough Division review of three program IUPs, a permit writer may
be approved to participate in this process. Permits submitted under this process are not subject to a full Division
review. Benefits to the POTW include less paperwork to submit and immediate approval of your permits.
6) Staff Assignments: Our goals for project review are 30 days for IUPs and SIU drops and 90 days for ERP,
HWA, IWS, LTMP and SUO. Last year we met our goal 99% of the time.

7) Workshops: The following workshops are planned for 2019. The invitations and directions for these workshops

space is limited at each site.
Pretreatment Annual Report (PAR) — January 24th (Raleigh) and January 29th (Salisbury)
Headworks Analysis (HWA) — January 30th (Salisbury) and April 11th (Raleigh). [HWA workshops may be re-
scheduled if not enough students sign up].
Industrial User Permit Writing (IUP) — May 22 (Raleigh)
8) Pretreatment Program Info Database Sheets: Please review your attached program info sheet(s) and historical
SNC sheet(s), if applicable, and make necessary updates to any of the information presented. Send the
corrected sheets back with your PAR, due March 1, 2019. If all the information was correct, please indicate
that in your PAR. Especially note the due dates as this may be the only reminder you get. Please note:
a. All pretreatment programs (full and modified) must submit a PPS form with their PAR per an EPA
requirement of the State.
b. Historical SNC sheets only list currently Active SIUs that had SNCs from 2011-2017.

9) Email Addresses: PERCS often uses email to send out mass mailings. To make this an effective and cost
saving tool, we ask that you keep PERCS updated on your current email address as well as which staff member
is the "primary" contact for your POTW - see "prim" field next to staff names on Program Info sheet, and email
field below names. For programs with more staff than the printed sheet can show, a separate contacts page
has been included. Please make any staff changes on that sheet and return to PERCS with the PAR.
