Dear Mr. Hepler:
On January 9, 2019, Paul DiMatteo of the Division of Water Resources (Division), Winston-Salem Regional Office
met with you, Operator in Responsible Charge (ORC)and Ronald Massey, backup ORC,to conduct a Compliance
Evaluation Inspection of the above referenced Wastewater Collection System.
This inspection consists of(1) a review of the permit and accompanying documentation to determine
compliance with permit conditions, and (2) an on-site inspection of several collection system components. The
attached inspection form notes the areas that were evaluated for the inspection, with notable findings
outlined as follows.
Site Inspections
1. Pump Stations—the Riverdale, Foxwood Meadow, and Barrington pump stations were inspected. Riverdale was
being bypassed because of equipment failure during one of the recent hurricanes.The City was actively looking
into upgrades and replacements.At Foxwood Meadow and Barrington,the audible and visual alarms did not
come on when tested, and one backup generator did not come on. Please be sure that AV alarms are
maintained as required.
2. Lines—The Deep River and Richland Creek aerial lines were inspected and no issues were observed.

1. Please be sure to document an annual general observation of the entire collection system. A certification
statement is acceptable, and an example statement was provided after the inspection.
