MEMORIAL DRIVE GREENVILLE,NC 27834 DB00408 Page 2 of 4 July 26, 2018 Completion Date (other than vegetation): November 4, 2018 Liquidated Damages: $600.00 per calendaz day Contract Review • Major Contract Items: Line Item No. 80 Prestressed Concrete Cored Slabs • Specialty Items: Line Item Nos.: 30-34 Guardrail; 38-62 Erosion Control and 63 Reforestation • Fuel Price Adjustment: Base index price for Diesel #2 Fuel is $2.0070 per gallon • The DBE Goal was met at 7% with Florence Concrete Products. • Cooperation Behveen Contractors: NCDOT Traffic Services will supply signs and barricades. A one-week notice to Traffic Services is required per Mrs. Lentine. • Certified Erosion Control/Stormwater Officer documentation was submitted by Mr. Wisniewski as well as all other DOT required submittals. Mc Beasley verified receipt of alI documents. • Mrs. Lentine advised no borrow pits aze listed on the contract but should it be needed advanced notice is required and must come from a commercial pit. • Lump Sum Grading is required with this conffact per Mrs. Lentine. • Contract Addendum 2 decreased the base price for asphalt binder for plant mix from $430.45 per ton to $429.00 perton. • Relocation of Existing Gate in accordance with plans was discussed by Mrs. Lentine and verification of that plan was given by Mr. Reeves. • Safety Fence and Jurisdictional flagging is required due to the wetlands per Mrs. Lentine. • Native Grass Seeding and Mulching shall be performed on the disturbed areas of wetlands per Mrs. Lentine. Mr. Blankenship added native seed mix is mainly used in wetlands. Mowing and maintenance areas use permanent. • Mrs. Lentine quoted Buffer and Stream Impacts in the Tar-Pamlico River Basin. Mr. Ward reviewed specific conditions of certification. No. 3: A dry work area shall be maintained to prevent direct contact with stream water during construction if concrete is used. No. 4: Highly visible fencing is required before any land disturbance at the outside buffer, wetland or water boundary. No. 5: No staging of equipment is permitted in the water or buffers during consWction. No. 6: Contamination prevention is required by regulazly inspecting mechanized equipment to avoid fluid spills when operating near surface water. No. 9: Erosion and sediment control practices must be in compliance per Mr. Ward. No. 11: Nylon Mesh Matting must not be used within 25 feet from top of stream bank for erosion control. No. 12: All erosion control devices mus[ be removed within two months of DLR area release. No. 17: The bridge demolition must be accomplished in strict compliance with the most recent version ofNCDOT's Best Management Practices for Construction and Maintenance Activities. No. 23: Drilling or driving within the srream channel, stream bank or within 5 feet of the top of bank requires a turbidity curtain. Mr. Reeves stated there is one interior bent. No. 28: Riparian azeas within the construction limits requires aative vegetation by the end of the growing season following completion of construction. No. 29: Dischazging Hydroseed mixtures adjacent to surface waters is strictly prohibited. No. 30: All activities must comply with the State water quality standards of the Clean Water Act or the NCDWR may reevaluate and modify the certification. DB00408 Page 3 of 4 July 26, 2018 No. 31: Any violation should be reported to the Division of Water Resources within 24 hours of discovery. No. 32: Upon project completion, the NCDOT Division Engineer shall completed and retum the "Certification of Completion Form" to notify NCDWR. No. 33: A copy of Water Quality Certification should be maintained on the construction site at all times. Mr. Ward stated stormwater pertnit compliance is required which would include weekly monitor forms, inspection forms, temporary stabilization requirements and covering of stockpiled materials. Mc Reeves stated mailboxes and rain gauges will be on-site. General Discussion Mr. Reeves reassured Mc Blankenship no gravel would be used in driveways. Mr. Blankenship stated no gravel can be used within the buffer zone. Mrs. Lentine questioned what would be used for the laydown area. Mr. Reeves responded, laydown area would be the road iuelf behind the crane. Per Mr. Blankenship, the contractor was advised not to go beyond the hatched area or permitted buffer. The safety fence needs to be brought to the access drive. Mr. Reeves requested that dirt removed during bridge construction be taken to the local DOT yard for storage. Mrs. Lentine requested Pitt Co. Maintenance be notified for approval. Mr. Beasley agreed to check with maintenance about storing excavated material. M& J Seeding will be responsible for erosion control and seeding per Mr. Reeves. Mrs. Lentine stated this project is federally funded with a special provision for the Davis-Bacon Act. However, certified payroll is not required because of project being on local roads. Minimum wage rates do apply per Mrs. Lentine. Mr. Wisniewski estimated a construction time of 12 %, - 13 weeks for compietion. Mr. Wisniewski requested Mr. Blankenship, Mr. Reeves and Mr. Peasley meet on Monday, August 13'h for stakeout. There being no further business, the meeting was adjoumed. These minutes are prepazed as they aze dowmented. Any discrepancies with the content of these minutes should be submitted in writing to this office within 7 days of receipt. Failure to do so will be an indication of agreement.
