Below is the Pretreatment Annual Report Narrative for the reporting period January 1,
2018 to December 31, 2018.
GENERAL INFORMATION:
GENERAL PROGRAM INFORMATION:
• AT, LTMP, HWA, SUO, EPR, IWS, and Permits are up to date.
• HWA was approved by the Division on 5720/16.
• AT was updated and approved along with the HWA.
• LTMP was updated and approved by the Division on 9/18/17.
• IWS was approved by the Division on 1/7/19.
• As per Division request, Surry County Landfill was issued a local IUP
effective 2/15/19
• SUO was updated and approved by the Division on 10/25/12.
• ERP was updated and approved by the Division on 10/25/12.
• IU Permits were issued 7/01/16. Permits were approved by the Division
on 5/19/16.
• Dates on the Division's Database Program Information Sheet are correct with the
exception of the IWS approval and due dates. A copy of our Program Information
Sheet is enclosed with corrections.
• No historical SNC report was received from the Division
• A copy of the PAR delegation letter is provided
GENERAL PERMIT INFORMATION:
• No permits lapsed or expired prior to renewal.
• No new SIU's
• Current permits are due to expired on 6/30/21.

SIU INFORMATION
There were no SNC violations for the City's only SIU for this reporting
period.

Dear PERCS Unit,
Section 1.1 and 1.2 (a) (25) of the City of Mount Airy's Sewer Use Ordinance designates
our Director of Public Works as the POTW Director and delegates responsibility for
overall operation of the City of Mount Airy's Pretreatment Program. It also states that any
powers granted to or imposed upon the POTW Director may be delegated by the POTW
Director to other City personnel. Therefore, I hereby designate the City's Wastewater
Treatment Plant Supervisor with the authority to sign all required documents under the
Pretreatment Program, including the authority to sign the Pretreatment Annual Report
(PAR) and submit it to NC DWR/PERCS Unit. Currently, the City of Mount Airy's
Wastewater Treatment Plant Supervisor is Mr. Christopher Marion.
