Davis -Martin -Powell is providing utility engineering services for NCDOT Project R-5775, which involves improvements for
the Intersection at US 321 (Hickory Blvd.) and SR 1109 (Pinewood Road Ext.) in Granite Falls.
The Town of Granite Falls has a section of an existing gravity sewer line that must he relocated due to conflicts with the
proposed roadway and storm drainage improvements. The project will involve the relocation of 211 LF of 8" gravity
sewer and associated appurtenances. There will he no changes to the flow conditions for the existing sewer main.
We are enclosing the following information in regards to the referenced project for your review and approval:
• Two (2) copies of the executed Fast -Track Application for Gravity Sewers, Pump Stations & Force Mains
• Two (2) copies of the 8.5"x11" color USGS quad map exhibit
• Two (2) copies of an 8.5"x11" street level map showing the project location
• Check # 063102 in the amount of $480.00 for the review fee

This application is fa sewer extensions involving gravity sewers, pump stations and force mains, or any combination that has been
certified by a profess' nal engineer and the applicant that the project meets the requirements of 15A. NCAC W and the Division's
Minimum Design Cri ria and that plans, specifications and supporting documents have been prepared in accordance with, ISA
NCAC 02T, 15A NC C 02T.030Q Division policies antl good ene'nee inspractices,
While no upfront an veering design documents we required for submittal, in accordance with 15A NCAC 02T .0305(b), design
documents must be p spared prior to submittal of a fast track permit application to the Division. This would include plans, design
calculations, and pro ct specifications referenced in ISA NCAC 02T .0305 and the applicable minimum design criteria. These
documents shall he av liable upon request by the Division.
