Stephen, per our phone conversation earlier today, attached is a CAMA Minor
Modification request for CAMA Major Development Permit No. 60-16, which authorizes
the replacement of Bridge No. 16 over the AIWW on NC 210/NC50 in Pender County;
TIP No. B-4929. This request addresses the contractors request to construct a work
trestle to assist in the demolition the of existing swing span bridge and design revisions
to Roland Ave. and the proposed cul-da-sac on the Island side of the existing bridge,
which was requested by the Town of Surf City.
Per conversations with the Corps and DWR, a modification of the 404 and 401 will not
be required for the items listed above.. The Resident Engineers office and contractor
will continue to coordinate with the local USCG regarding any required updates to the
maritime notice.

The NCDOT is requesting a Minor Modification to CAMA Major Development Permit #60-16. The contractor has
proposed to construct a work trestle on the mainland end of the existing swing span bridge to assist in its demolition.
The existing bridge structure will not support the 275-ton crane that is required to dismantle the concrete center pivot
of the swing bridge. The proposed work trestle will be a 2-span structure that is 110 ft. x 30 ft. and will require the
installation of nine pipe piles. Two additional pipe piles will need to be installed on the North side of the trestle for
vessel mooring. The work trestle will be constructed within the footprint of the existing structure.
The Town of Surf City has also requested that the Department make a few design revisions to Roland Ave. on the
island side of the existing bridge. The design changes include, moving the proposed cul-da-sac approximately 125 ft.
to the north of the current proposed location, provide additional parking along Roland Ave. and add a 5 ft. sidewalk to
the east side of the roadway. All working will be within the existing right of way or easements and there will be no
impacts to jurisdictional resources associated with these changes.
NCDOT will continue to follow all other conditions of the CAMA permit and subsequent modifications as authorized.

Please see the follow demo plan for the existing swing span bridge, the plan will be broken down by side but
operations will run concurrently. Balfour Beatty will us NCDOT BMP's for all aspects of the demo
operations.
Mainland Side
- Disconnect all utilities servicing any of the mainland side bridge facilities.
- Removal all overhead signals and traffic limiting devices as you approach the bridge end bent.
- Open bridge and install one span of trestle from the end bent towards the swing span center
pivot. (see attached drawings)
- Assemble sectional barge approaumately 40' x 90' in water adjacent to trestle.
- Load 70 ton crane and misc. materials on barge.
- Remove matts and trestle beams to allow bridge to close.
- Once closed the barge mounted crane will load beams on existing fender system to shore up
swing span for demolition.
- Reopen bridge, shore swing span to fender system, and install second trestle span. 275 ton
crane will be moved out onto trestle.
- The 70 ton crane on the barge will handle demo material on the
South side under the powerlines and the 275 ton crane on the trestle will handle demo materials on
the North side of the bridge structure.

- Demoed materials will be consolidated on the mainland approach for processing and transport.
- Once the steel structure is removed the removal of the concrete center pivot will begin. All
materials will be consolidated on the mainland approach for processing and transport.
- Once the concrete center pivot is removed the concrete support pile will be removed.
- Following the removal of all concrete bridge materials the 275 ton crane will begin fender
system removal within its reach and the crane on the barge will remove all other fender materials.
- Once the demo operations are completed the 70 ton crane will be removed from the barge and
the barge and trestle spans will be removed.
Island Side
- Disconnect all utilities servicing any of the island side bridge facilities.
- Removal all overhead signals and traffic limiting devices as you approach the bridge end bent.
- Cut the bridge tender house foundation piling and move as one complete unit to the recently
closed portion of Roland Ave for asbestos abatement operations.
- Start existing deck removal with 110 ton crane, all demoed materials will be consolidated on
the closed portion of the roadway for processing and transport.
- Remove all caps and piles moving from the channel towards the island end bent.
- Once the crane on the barge completes removal of the steel structure on the mainland, it will
move to the island to begin fender system removal.
