The inspections included records review and residuals processing review of the Fourth Creek WWTP facility. A review of the storage at the Fourth Creek WWTP was performed as part of the inspection. The MRO would like to thank Kenneth Galloway for his time during these inspections. Permit WQ0006245: Log entries for the lime stabilization analysis and cross-referenced records for the pH meter calibration were well organized. O&M records were reviewed and appear to be well organized and up to date. During the site visit, there was a limited amount of residual material present on the Fourth Creek storage pad area. The storage pad at the fourth creek facility is experiencing issues with the product becoming saturated due to high precipitation. This concern is addressed further in the BIMS report. Please request that Prism Laboratories report the SAR calculations with the variables populated, rather than presenting the general formula. This will allow for quick verification of these values. This concern was addressed during the site visit. The corrected analytical reports are expected to be present in the Annual Report for this permit. This permit expires on September 30, 2020. Please be sure to submit the permit renewal application at least 180 days prior to the permit expiration date. Confirmation was made that dedicated land for this permit continues to be in place. The permittee was reminded that if the dedicated land was removed from available use, this permit may become invalid. This permit expires on February 28, 2019. A renewed permit has been issued and it will become effective on March 1, 2019. A file review of the O&M and analytical data for the Third Creek WWTP residuals was reviewed during this site visit. Please update all contacts and ORC's and persons to be associated with these permits. Changes can be made by submitting Water Pollution Control System Operator Designation Form as stipulated in NCAC 15A 8G .0201. A copy of the link has been attached to this email. Overall the facilities were found to be compliant with the permit. The attached reports should be self-explanatory.
