Dear Mr. Konsul:
On January 3, 2019, Paul DiMatteo of the Division of Water Resources (Division),Winston-Salem Regional Office
met with Andrew Phillips, Operator in Responsible Charge (ORC),and Mark Haver, Backup-ORC,to conduct a
Compliance Evaluation Inspection of the above referenced Wastewater Collection System.
•
This inspection consists of(1) a review of the permit and accompanying documentation to determine
compliance with permit conditions, and (2) an on-site inspection of several collection system components.The
attached inspection form notes the areas that were evaluated for the inspection, with notable findings
outlined as follows.
Site Inspections
1. Pump Stations—the Avenbury Circle and Kynwood Pump Stations were inspections and no issues were noted.
2. Lines—The area of the Avenbury Circle High Priority Line was observed and no issues were noted.
3. Manholes—A manhole near the Kynwood Pump Station was inspected and no issues were noted.
Permit Conditions
1. There have been no reported Sanitary Sewer Overflows(SSOs)since the original permit issuance.Operators
were adequately familiar with SSO response and reporting requirements.
2. Please respond to this letter with a copy of the most recent Capital Improvement Plan (See Condition 1.6). Please
provide the copy by mail at the address in the letterhead, or by email to the inspector at
paul.dimatteo@ncdenr.gov.
3. Please ensure that a copy of the Contingency Plan for Pump Failure is available at each pump station (Condition
1.8).

Inspection Summary: •
I met with Andrew Phillips,ORC,and Mark Haven,backup-ORC,to complete the inspection.
-There have been no reported SSOs in this collection system since the original permit issuance.We reviewed response and - - -
reporting requirements.
-There have been no system modifications or extensions.
-Last year,a crew was hired to take pictures of the inside of each manhole as well as GPS coordinates.They said they
plan to compare manhole conditions year by year and use the coordinates to create a new GIS map.
-A copy of the most recent Capital Improvement Plan was not available during the inspection.Qualitatively, ORCs said that
planned improvements include the aforementioned GIS map and one new grinder pump in each pump station.
-A copy of the Contingency Plan for Pump Failure should be kept at each pump station(Condition 1.8)

Comment: Map is a Google Earth overlay that includes manhole locations and labels, approximate pipe
ages, material, and sizes. Pump capacities are available. Operators said they were going to
move to a full GIS program with more accurate measurements.The remaining required items
should be included on that map

Comment: Access to this pump station is restricted by a fence and locks, as well as locks on the
equipment. The grounds of this pump station also holds 2 portable generators.This pump
station is equipped for quick connect AV alarms were verified; ORC said that he was on call that
day so the SCADA system did not dial him.
