Sorry for the delayed response on this. Per the requested items in your email, please see my responses below:
I did confirm with Mike Anderson of Advantage Civil Engineering that the temporary crossing will be a culvert
type crossing and not a ford type crossing. I also asked Mike Anderson of Advantage Civil Engineering that he
ask the contractor to carefully read the PCN, this email and the 404/401 permit when it is received.
As stated in the PCN, flow in the tributary will be pumped around the work area during installation of the sewer
line and culvert type temporary crossing. Flow will be returned to the streambed after the sewer line is
installed and the temporary culvert is in place. During removal of the culvert type temporary crossing and
installation of the permanent arched culvert, stream flow will again be pumped around the work area. At
completion of the permanent crossing care will be taken to maintain and mimic natural channel geometry
above and below the crossing. Floodplain benches will be installed and graded to blend into existing benches if
they exist in undisturbed areas above and below the work area. Disturbed streambank slopes will be graded to
a maximum slope of 3H:1V and blend to natural slopes above and below the work area. Disturbed areas of the
streambank will be seeded with a Mountain Riparian Seed Mix similar to Ernst NC Mountains Riparian Mix
(ERNMX-304) and an annual grain appropriate for the season (oats, rye or millet) per manufactures
specification. All disturbed areas of the streambank within the trout buffer shown on the plans will be
fertilized using care to keep fertilizer out of the water/stream and mulched with wheat straw. Areas of the
streambank below the ordinary high water mark or bankfull elevation will be reinforced with minimum 400
gram/square meter weight Coir slope matting and be installed per the manufactures specification for
streambank applications. During periods of dry weather, streambanks will be watered until a six inch stand of
grassy vegetation is established.

Joel,
On December 17, 2018, the Division of Water Resources (Division) received your application dated December 14, 2018,
requesting a 401 Water Quality Certification from the Division for the subject project. The Division has determined that
your application is incomplete and cannot be processed. The application is on hold until the following items are
addressed:
The Temporary Stream Crossing Detail on Sheet D3.2 of the Pre -Construction Notification (PCN) depicts both
ford and culvert type crossings. Please confirm the temporary crossing will be a culvert type crossing or clearly
explain why the culvert type crossing is infeasible. [15A NCAC 02B .0200]
2. Please explain how streambanks will be stabilized following sewer line installation and/or provide a construction
sequence for the sewer line stream crossing, temporary stream crossing, and arch culvert that details how
sedimentation to the stream channel from disturbance along the streambanks will be prevented. If applicable, a
streambank stabilization detail may be advantageous. [15A NCAC 02B .0200]
Pursuant to Title 15A NCAC 02H .0502(c), the applicant shall furnish all of the above requested information for the
proper consideration of the application. If all of the requested information is not received, the Division will be unable to
approve the application and it will be returned.
