DANIEL SMITH Interim Director Karen Ledford Oak Valley Hardwoods 8482 US 221 North Marion, NC 28752 Subject: Compliance Inspection Permit No. NCG 210464 NORTH CAROLINA Environmental Quality January 24, 2019 Oak Valley — Pilot Mountain Surry County Dear Permit Holder: On January 24, 2019, Brandon Wise, inspector with the North Carolina Department of Environmental Quality (NCDEQ) — Division of Energy, Minerals, and Land Resources met with you and Randy Lawless at the facility located at 175 Tree Cut Lane in Pilot Mountain, to conduct a compliance inspection as part of the comment period for a new permit application. The inspection consisted of review of the Stormwater Pollution Prevention Plan (SWPPP) and each of its components, review of the facility outfall, as well as the review of the overall site conditions. The compliance inspection was conducted to ensure all site practices and facilities are in working order. The inspection was also conducted as assistance to Oak Valley Hardwoods should there be any questions about the implementation of the permit NCG210464. Permit: This facility has General Stormwater Permit NCG210464 to discharge stormwater from industrial activity associated with Lumber and Wood Products, Except Furniture [SIC 24] under the National Pollutant Discharge Elimination System (NPDES). The permit is not yet effective as the inspection was completed as part of the comment period. Records/Reports: This facility is required to develop and maintain a SWPPP in accordance with Part II, Section A of the permit. All qualitative and analytical monitoring records are required to be maintained with the SWPPP for a minimum of five (5) years. Under this permit, analytical monitoring is required under current operating procedures. The facility records do not include any reports as the facility was not actively permitted prior to the inspection. The SWPPP for this facility was very well put together and had been implemented prior to the inspection. The facility is required to maintain employee training records in accordance with Part II, Section A, Item 6 of the permit. Ensure that these records are added to the SWPPP when training is completed. DANIEL SMITH Interim Director NORTH CAROLINA Environmental Quality Facility Site Review: This facility operates as a saw mill and does not perform any other treating of wood on - site. Logs brought on site are stored in the northern section of the site, with the actual mill being located on the western edge of the site. Just east of the mill, cut wood is stored prior to being shipped off site. The southern portion of the site is where the office and parking areas are located, no industrial activity occurs on the southern portion of the site. At the time of the inspection the area near the log storage had a large amount of sediment cover on top of the gravel drive. This area should stay cleaner as keeping sediment out of stormwater is part of controlling pollutants from the site. A single outfall is located on the eastern edge of the northern side of the site. All mill and stored wood stormwater converges at this outfall, which will be the sampled outfall in the future. The stormwater from the office building and parking area converges at an outfall prior to reaching the industrial part of the site. Should industrial activities or storing of product extend to this part of the site sampling may be required at the southern outfall. Effluent Receiving Waters: Effluent from this facility drains into an un-named tributary of Flat Shoal Creek, class "C" waters of the Yadkin Pee -Dee River Basin. Self -Monitoring Program: This facility is required to provide Qualitative and Analytical Monitoring twice annually. The single outfall at the northeastern side of the property will be properly maintained and all monitoring will be performed there. There are no comments about any changes to the issued permit.
