REGAN Seurerary LINDA CULPEPPER January 2, 2019 Internn Direcror ry Mr. Tommy Spicer Wilkes County Schools 613 Cherry St. North Wilkesboro, NC 28659 Subject: Notice of Deficiency — (NOD- 2019-PC-0001) Compliance Evaluation Inspection Trap Hill Elementary School WWTP NPDES Permit # NCO046426 Wilkes County Dear Spicer: A Compliance Evaluation Inspection was performed by Division of Water Resources, Winston Salem Regional Office, staff person Justin Henderson, on December 19, 2018. Mr. Tommy Spicer, Operator in Responsible Charge (ORC), was present for this inspection. This type of inspection consists of two basic parts: an in -office review of the facility files and self -monitoring data and an on -site inspection of the facility. This inspection reflects noncompliance with Permit No. NC0046426, the following deficiency requires your attention and action: 1) Failure to properly maintain the surface sands filters. Excessive vegetative growth was observed in both surface sand filters. Maintenance shall be performed at a frequency that will prevent ponding of effluent and/or vegetative growth per Permit Condition Part II. Section C. (2.) which states the Permittee shall at all times properly operate and maintain all facilities and systems of treatment and control. Failure to perform proper maintenance to prevent vegetative growth in the sand filters is a repeat deficiency that was previously documented in a Compliance Evaluation Inspection report dated December 8, 2017. Along with the above described deficiency, the following items of concern also require your attention: 1 a) One siphon has been removed from the dosing tank. Removal of this siphon has nullified the alternation of surface sand filters as part of the treatment path. Therefore, the "back" surface sand filter has been rendered inactive. If treatment equipment will not be replaced, then permit modification shall be requested to accurately describe the active treatment equipment and operations. The following deficiency requires your attention: 1) Failure to perform proper operation and maintenance of all treatment and control facilities. Excessive vegetative growth was observed on the surface sand filters. This is a repeat deficiency that was previously documented in an inspection report dated December 8, 2017. Along with the above described deficiency the following items of concern also require your attention. a) One siphon has been removed from the dosing tank therefore preventing the alternation of surface sand filters within the treatment path. This situation has effectively renedered the "back" sand filter inactive. If treatment equipment will not be replaced, a permit modificaton shall be requested, and the treatment equipment updated to accurately reflect the treatment path. b) Post -calibration records for the pH meter were unavailable. Post -calibration shall be performed and documented per use, and made available for review upon request.
