The form said the Facility name was "Reagents," which was the only change from before, aside from contact changes. If the facility name wasn't supposed to change, were you just trying to update the Owner Affiliation / contacts info? We have a different form for Owner Affiliation Only changes. Do you think you submitted a Name/Ownership change form by accident? That form is for company ownership changes and facility name changes. Is the Facility name supposed to be "Reagents" or stay "Reagents Holdings LLU as before?
