Attached is a draft of the subject DWR form.
There is a problem with the form for the two email entries (bottom of Page I and top quarter of Page 2). As you
will see, our email addresses are a couple letters too long to fit in the available block, which results in the email
address wrapping around to a second line, which results in the "Phone" and "E-mail Address" under the last line
on Page I to be taken to the top of Page 2. It also results in the 2-page form becoming a 3-page form.
Ironically our old email extension of "@dupont.com" fits in the space, but our new extension of
"@chemours.com" does not.
If your IT staff could modify the form and merely increase the size of the email data entry block, then things
would fit. There is adequate room, since the only other thing on that line is a telephone number.
I will deal with the issue via a pen so that I can submit the change in facility name to DWR.
