The attached Table 1 reports the on -site groundwater monitoring results for the compound
perfluorooctanoic acid ("PFOA") from the sampling event in September 2015. An aerial map of
the facility is attached showing the locations of the sampled wells and their analytical results.
The shallow well (SMW-05) located north of and adjacent to the PPA Manufacturing Facility and
the temporary piezometer (INSITU #2 SHALLOW) located north-northeast of the PPA
Manufacturing Facility and near the facility's northern boundary, were both found to be dry and
could not be sampled.
