'Beginning in April 2001 and ending in August 2001, samples taken for HFA-hydrate analysis were either Outfall 001 grab samples or were from the discharge of the temporary alkaline/thermal treatment system. Therefore, enclosed are amended DMRs for the months of April 2001 through August 2001 that report the actual HFA-hydrate monitoring that occurred. It must be pointed out that throughout this period of time, monitoring for HFA-hydrate did occur as a grab sample on virtually a daily basis at either Outfall 001 or at the discharge of the alkaline/thermal treatment system. All of these analyses had no detectable levels of HFA-hydrate. DuPont is very confident that given the effective HFA-hydrate destruction of the treatment facility, there is no possibility that a detectable HFA-hydrate concentration could leave the alkaline/thermal treatment facility, let alone being detectable at Outfall 001. On February 16, 2001, the Division of Water Quality authorized the DuPont — Fayetteville Works to construct and operate a temporary alkaline/thermal treatment facility that would remove all measurable concentrations' of HFA-hydrate. The authorization included a requirement that DuPont monitor for HFA-hydrate as a grab sample every four hours at Outfall 001. Because of this around the clock monitoring requirement, sample collection was performed by the Nafion® personnel who were operating the alkaline/thermal treatment facility as opposed to the site's ORC or backup ORC, who were not continuously present during all the operating hours of each day. E. ! du Pon( tie Meniours and Company R-4 h, c. 7 `tlpfi DEQ-CFW 00063133 Mr. Paul Rawls NCDENR DWQ December 13, 2001 Page 2 of 3 At the request of DuPont, in a letter dated April 3, 2001, the Division of Water Quality approved a change of monitoring for HFA-hydrate to weekly composite sampling of Outfall 001. This monitoring change was not properly integrated into either the site's Wastewater Treatment Plant (WWTP) procedures or the alkaline/thermal treatment facility's procedures. Through a recent investigation conducted by DuPont, it was determined that involved personnel made erroneous assumptions as to who would be responsible for the monitoring. The site's ORC assumed that the Nafion® personnel would continue the HFA-hydrate sampling since they had that responsibility when the monitoring was every four hours. The Nafion® personnel assumed that the ORC would do the composite sampling since he is responsible for all other composite sampling. In April 2001, when the alkaline/thermal treatment facility was started up after the initial cleanup of the WWTP, the Nafion® personnel began sampling Outfall 001 as a single daily grab sample. With only a few exceptions, a grab sample of Outfall 001 was taken each day from April 9 until June 21, 2001. In addition, a composite sample was taken on April 12, May 3, and May 24, 2001, when the ORC took the Outfall 001 sample at the request of the alkaline/thermal treatment facility's personnel. The attached amended discharge monitoring reports for Outfall 001 lists the analytical results of the HFA-hydrate monitoring. The DMRs list the results as being from either a composite or a grab sample. On June 22, 2001, responsibility for the HFA-hydrate analysis was changed from the Nafion® research laboratory to the Nafion® manufacturing laboratory. With this change, the alkaline/thermal treatment facility's personnel were instructed to cease the daily monitoring of Outfall 001 due to the belief that the ORC was taking the required Outfall 001 composite sample. However, the personnel continued taking the daily grab sample from the discharge of the alkaline/thermal treatment facility's final Quality Tank. This discharge would have had the highest possible concentration of the treated HFA-hydrate due to no dilution with other wastewaters at that point. The Quality Tank discharge was ultimately introduced into the Equalization Basin of the WWTP for normal biological treatment. The attached table reports the analytical results from the daily grab samples of the alkaline/thermal treatment facility's Quality Tank. In all cases, there was no detectable HFA- L' hydrate leaving the treatment facility. Again, these samples were the undiluted discharge of the treatment facility and would have the highest possible concentration of HFA-hydrate. During the period from June 22 to August 31, 2001, only two days (July 4 and 5) were apparently not sampled for HFA-hydrate. All other days were sampled and analyzed, with the result being no detectable HFA-hydrate concentration. DEQ-CFW 00063134 Mr. Paul Rawls NCDENR DWQ December 13, 2001 Page 3 of 3 Since September 2001, the site's ORC has been sampling the Outfall 001 composite sampler for the HFA-hydrate analysis. Therefore, beginning in September 2001, all monitoring of HFA-hydrate has been correctly done at Outfall 001 as prescribed by our modified NPDES permit requirements. Also, all DMRs submitted to DWQ beginning with and subsequent to September 2001 have been correct. DuPont regrets this incident and assures the Division that steps have been taken to prevent any recurrence of this nature. The entire situation surrounding the HFA-hydrate discovery in the WWTP effluent and the subsequent monumental effort of the WWTP cleanup and management of the temporary treatment system, with the involvement of employees from various areas of the site, unfortunately lead to this incident. However, proper procedures are now in place and the daily operation of the WWTP has returned to normal, so samples are being taken correctly, pursuant to the requirements of the site's NPDES permit as amended by the April 3, 2001, letter from the Fayetteville Regional Office. The analytical result for the Outfall 001 BOD 5-dav analysis for April 24. 2001. was reported by the laboratory to this site as being "estimated 3.8 mg/L BOD". This result was due to the failure of the test dilutions to meet the required 2 mg/L dissolved oxygen depletion. Per Instructions by DWQ (Bob Sledge), this result was used to calculate a mass discharge from Outfall 001, and that mass discharge result was Included in the monthly average. See the attached letter from Simalabs International detailing this analytical result.
