Dear Mr. Johnson:
A review of subject self -monitoring report for the month of OCTOBER 2002 has been completed by
the Fayetteville Regional Office. This review indicated the following violation(s) of the following
parameter(s):
MISSING DATA FOR A BOD AND A TSS RESULT FOR THE WEEK OF OCTOBER 6.
Please be advised that the Division of Environmental Management may pursue enforcement action
for this and any additional violations of State Law; however, the quick notice to our office and the subsequent
remedial actions taken by you are seen as positive steps to maintaining compliance.

Mike Johnson called on 12/3/02 to notify us that he had exceeded his
monthly BOD for November based on the results he had received from his
commercial lab. A sticky slime entered the plant and appeared on 1/2 -
3/4 of the basin. They believe it was a change in a chemical usage with
in the Albicite facility. The supplier for a particular polymer was
changed beginning the first of November. The exceedance began November
18, 19 and 20 and then the next week 25, 26 and 27. Mr. Johnson called
today 12/5/02 to notify us that his final BOD result for the month was
460 lbs/day. Their permit limit is 190.3 lbs/day and their monthly
average is 229.9 lbs/day. They have began adding a polymer in their EQ
basin to assist with settling slime out and it appears to be working so
far based on how the clarifiers are }pack to normal.
