Attached are 2 spreadsheets (Permits Issued and NOVs Issued) reflecting the permits and NOVs the Water Quality
program has issued since January 1, 2011, through October 21, 2015, when the data was pulled from the data system.
Within each workbook is a worksheet showing the summary data of the permits or NOVs issued. These tabs are
followed by worksheets showing the raw data generated from BIMS or other data source.
For the NOVs issued the column indicating NOV Resolution reflects NOVsthat were escalated to civil penalty
assessments. A footnote is indicated on the affected worksheet tabs.
