In accordance with your permit major modification request received October 12, 2018, and
subsequent additional information received December 11, 2018, we are forwarding herewith a modified
Attachment B certified December 21, 2018, to Davie County for the subject residuals management program.
Modifications to the subject Attachment are as follows: Addition of ten fields in Davie County
totaling 176.2 acres, those fields being NC-DV-008-004 to -008; and NC-DV-012-01 to -08.
The modified Attachment B shall be effective from the date of certification until April 30, 2022
(i.e,, expiration date of the current permit), shall void Attachment B of Permit No. WQ0010583 issued May
19, 2017, and shall be subject to the conditions and limitations as specified therein, as well as those specified
in the above -referenced permit. Please replace Attachment B of Permit No. WQ0010583 that was issued
May 19, 2017 with the modified Attachment B. Please note that the Permittee shall submit a renewal
application no later than November 1, 2021.
Please note that only the maps for new land application sites are attached to this letter (i.e., Figures
3, 4, 5, and 6). It is the Permittee's responsibility to ensure that a complete permit is created by inserting
these documents into the previously issued permit.
If any parts, requirements or limitations contained in this certification are unacceptable, the
Permittee has the right to request an adjudicatory hearing upon written request within 30 days following
receipt of this permit. This request shall be in the form of a written petition, conforming to Chapter 150B
of the North Carolina General Statutes, and filed with the Office of Administrative Hearings at 6714 Mail
Service Center, Raleigh, NC 27699-6714. Otherwise, this certification shall be final and binding.

6. Because this entire field lies within the 100-year floodplain and the seasonal high water table (SHWT) is 18 inches below the surface, residuals must be injected or incorporated at a depth of 6
inches from the surface.
