A file review noted an error in the issuance date of the permit. The permit was originally sent to Public Notice in August 2018. However, the actual Notice was not published until October 31, 2018 [copy of affidavit is attached]. Per 15A NCAC 02H.0109 (a) (1), the Division cannot issue a permit sooner than 45 days after circulation of Public Notice. NC0055701 should have been issued on December 15, 2018, rather than November 15, 2018. Accordingly, we are forwarding the corrected pages for NC0055701. Insert the corrected pages and discard the ones previously sent to you.
