Sigmon - 713 Cascade Rd • Eden, NC 27288 SUBJECT: Compliance Evaluation Inspection Report and Notice of Deficiency Certificates of Coverage NCG550371 and NCG550372 NPDES General Permit NCG550000 _ NOD Tracking Number NOD-2018-PC-0331 Facility Located at 713 Cascade Ave, Rockingham County Dear Mr. Sigmon: On October 15, 2018, Paul DiMatteo of this office conducted Compliance Evaluation Inspection on the subject wastewater treatment facilities, one for the main house (NCG550372) and one for a rental house on the same property (NCG550371). i The systems each consist of a septic tank, subsurface sand filter, chlorinator, effluent pump and shared outfall with final disposal to an unnamed tributary to Dry Creek, class C waters in the Roanoke River Basin. No sewage malfunction was observed at or around the septic tanks. The outfall was not located during the inspection. At the main system, chlorine was not present in the chlorinators and a bypass pump was set up in the effluent pump tank and terminated in the nearby field.'Please note that the Permit requires that"the Permittee shall at all times properly operate and maintain all facilities and systems of treatment and control (and related appurtenances) which are installed or used by the Permittee to achieve compliance with the conditions of this permit." (Part II, Section C.2). Therefore, the effluent pump must be repaired, and an adequate supply of wastewater strength chlorine tablets must be maintained in the chlorinators. During a follow up conversation on December 3, you said that you would have an electrician repair the control panel once the weather allowed. Please contact the inspector once the repair has been made. Please also note that the General Permit requires annual effluent monitoring as specified in Part I.C. The General Permit was renewed effective October 23, 2018.A copy of the most recent Technical Bulletin is enclosed. A copy of the General Permit is available by searching on our website ( https://deq.nc.gov/about/divisions/water-resources ) or can be obtained by contacting the inspector. You will be contacted by the Division through separate correspondence regarding the status of renewal for your Certificate of Coverage. During the pre-inspection interview, he said: -No recent issues -He may need to replace a pump soon -Chlorine is added periodically -Septic tank was pumped 2 years ago -He knows where the discharge is - He does see water at the discharge periodically - He does not conduct annual sampling Site visit conducted on 10/15/2018. There are 2 systems on this property-one for the main house, and _ one for the guest house. For this system (main),there was no chlorine in the chlorinators, and there was some sort of hose coming out of the effluent pump tank that led to the nearby field. I searched around the staff report's GPS coordinates for the outlet pipe and could not find it. I spoke to Mr. Sigmon after the inspection, and he said the hose was temporary to pump the wet well down since the pump had gone out. Permitted discharge point is apparently somewhere in the field.
