The only items that I intended to change are the contacts and the ownership. Reagents use to be owned by Ricca
Chemical Company in Arlington, TX. (which is the reason for the contact change). We are no longer owned by Ricca.

I'm afraid I'm confused. The form said the Facility name was "Reagents," which was the only change from before, aside
from contact changes. If the facility name wasn't supposed to change, were you just trying to update the Owner
Affiliation / contacts info? We have a different form for Owner Affiliation Only changes. Do you think you submitted a
Name/Ownership change form by accident? That form is for company ownership changes and facility name changes.
Is the Facility name supposed to be "Reagents" or stay "Reagents Holdings LLU as before?

The official name is Reagents Holdings LLC. We actually go by both, but the official name is Reagents Holdings LLC
apologize if there is a difference and for the confusion.
The P.O. box is our mailing address. We have a lot of important documents that are either stolen and/or do not arrive to
us when only the street address is provided so I wanted to make that distinction if at all possible.

I have just been working on this submitted Name/Ownership change. I will send you a database report tomorrow once
the data refreshes to confirm the changes. The change appeared to be to the facility name only (from Reagents
Holdings LLC to just "Reagents"). I also noted that the facility address we had was just the P.O. Box, and I corrected that
to the street address. Contacts have also been updated.
