In the process of reviewing your request,the City of Burlington modified your permit in regards to the effluent limits and monitoring by Burlington. The following changes go into effect January 1, 2019: • The Combined Wastestream Formula was updated using actual process flow data, which slightly increased all corresponding effluent limits. • Monitoring frequency for Indulor has not changed. The City of Burlington will reduce sampling volatile and semi-volatile organic compounds from semi-annually to annually. Burlington's monitoring of all other parameters will remain semi-annual. • Special Condition#6 was added to allow for estimation of the amount of boiler blowdown discharge. Monitoring frequency by industry should be completed within the second Quarter and fourth Quarter of the calendar year. The POTW will monitor within the first Quarter and third Quarter of the calendar year. Other parameters and limits are also subject to change as a result of updated headwork's information. 5 Industrial User Pretreatment Permit (IUP) PART III Special Conditions 1. Slug/Spill Control Measures The permittee shall provide protection from accidental and slug discharges of prohibited materials and other substances regulated by this permit. The permittee shall develop a written slug/spill control plan and submit it to the Control Authority on or before January 15, 2017 for the approval by the Control Authority. The plan shall include,but is not limited to: Description of discharge practices, including non-routine batch discharges; Description of stored chemicals; Procedures for immediately notifying the POTW of slug discharges that would cause a violation of 40 CFR 403.5(b), with procedures for follow- up notification within 5 days; And if necessary, procedures to prevent adverse impact from accidental spills, including inspection and maintenance of storage areas, handling and transfer of materials, loading and unloading operations, control of plant site run-off, worker training, building of containment structures or equipment, measures for containing toxic organic pollutants (including solvents), and/or measures and equipment for emergency response. 2. Sludge Management Plan Ninety days prior to the initial disposal of sludge generated by any pretreatment facility, the permittee shall submit a sludge management plan to the Control Authority. 3. Flow Measurement Requirements The permittee shall maintain appropriate discharge flow measurement devices and methods consistent with approved scientific practices to ensure the accuracy and reliability of measurements of the volume of monitored discharges. Devices installed shall be a continuous recording flow meter capable of measuring flows with a maximum deviation of less than 10% from true discharge rates throughout the range of expected discharge volumes. The devices shall be installed, calibrated, and maintained to ensure accuracy. The meter must have a non- resettable totalizer that reads in cubic feet or gallons. There shall be no by-pass capability of the metering device. The meter shall be calibrated at a minimum of once per calendar year by an independent contractor. Modifications to the flow metering equipment shall be approved by the Control Authority prior to installation. If a required flow measurement device fails, the Control Authority shall be notified within 24 hours. 14 Industrial User Pretreatment Permit (IUP) PART III Special Conditions 4. Certified Laboratory Analysis Pollutant analysis shall be performed by a State of North Carolina Department of Environment Quality and the Division of Water Resources Certified Laboratory that is certified in the analysis of the pollutant in wastewater. 5. Equivalent Concentration Limits This SIU is subject to 40 CFR 414, and has been approved for Equivalent Concentration Limits as allowed by 40 CFR 403.6(c)(6). In order to demonstrate dilution is not being used, and to remain eligible for the Equivalent Concentration Limits, the following conditions must be met. a) Continue appropriate effluent flow monitoring. The effluent process flow meter readings shall be recorded daily and the daily effluent flow calculated in gallons per day. b) Submit monthly flow summary by the 20th day of the following month, with an analysis of flow and production rate changes to demonstrate no dilution. Increases in flow not accompanied by a corresponding increase in production may be an indication of dilution will be investigated and could result in disqualification. 6. Combined Wastestream Formula Flow Condition The permittee shall have available flow estimate methods to estimate the total daily volume of the boiler blowdown discharged to the pretreatment system. At the time of issuance of the permit, this method consists of general process knowledge that no more than 200 gallons is purged per blowdown, and the frequency shall be daily (5 days a week, Monday through Friday). The data shall be collected and reported as required in Part II, 1-4 of this IUP. Modifications to the flow estimate methods shall be approved by the Control Authority prior to the change. • 15 IUP Synopsis A.
