attached workplan drawings (1), dated "Revised 10/8/18".
This permit, issued on November 7.2018 1 , is subject to compliance with the application (where consistent
with the permit), all applicable regulations, special conditions and notes set forth below. Any violation ofthese terms may
be subject to rimes, imprisonment or civil action; or may cause the permit to be null ana void.
1) Unless specifically altered herein, this Minor Modification authorizes the modification of Condition
No. 20 of the original Permit No. 126-09, which was issued as amended on 5/8/15, all as expressly
and specifically set forth in the attached letter and workplan drawings. Any additional development
activities may require a modification ofthis permit.

This permit action may be appealed by the permittee or other
qualified persons within twenty (20) days ofthe issuing date.

ADDITIONAL CONDITIONS
2) In order to satisfy concerns ofthe resource agencies and in accordance with commitments made by the
permittee, waterfront lots to the east ofthe community area boat ramp access, along Plantation Circle
(lots 42-34), Windsong Point (lots 20-15), Mayfaire Drive (lots 51-49), and Flagship Drive and Flagship
Court (lots 48-42), shall have the existing deed restriction modified to allow for the ability to apply for
shoreline stabilization by way ofthe Division of Coastal Management's Major Permit application
process only. Furthermore, the deed restrictions shall prohibit the ability for the lots to apply for
shoreline stabilization by way ofthe Division's General Permit process.
3) Condition No. 20 ofthe original permit, which issued was amended on 5/8/15, remains in force for all
remaining waterfront lots not detailed in Condition No. 2 ofthis Minor Modification.
4) This Minor Modification shall be attached to the original Permit No. 126-09, which was issued as
amended on 5/8/15, as well as all subsequent modifications, refinements, and renewals, and copies of all
documents must be readily available on site when Division personnel inspect the project for compliance.
5) All conditions and stipulations of the active permit remain in force unless specifically altered herein.
NOTE: The U.S. Army Corps of Engineers assigned the proposed project COE Action Id. No. SAW -
2009 -01119.
NOTE: The Division of Water Resources assigned the proposed project DWR Project No. 06-1245 V4.
NOTE: A minor modification application processing fee of $100 was received by DCM for this project.
