The applicant must provide a monitoring report to the Corps of Engineers every six months, for a period of no more than five years (or until considered successful in writing by the Corps of Engineers), beginning once stream realignment work is completed according to the plans referenced in item # 1 above. The monitoring report must include photographs showing the entirety of the stream realignment reach from both upstream and downstream angles, as well as each structure, and any areas showing signs of instability. 3. If greater than 10% of the streambank area within the stream realignment reach is unstable (i.e. shows signs of erosion) at the end of the monitoring period, the permittee will be required to submit a contingency plan to correct the instability or compensate for the loss of stream function associated with the authorized stream channel fill, subject to the approval of the Corps of Engineers.
